چکیده

Gastro-oesophageal reflux (GOR) is defined as the involuntary passage of gastric contents into the oesophagus, and is a common cause of morbidity in childhood. GOR is an occasional physiological event in normal adults and children,1 2 but becomes pathological when its intensity or frequency increases or when complications arise (for example, oesophagitis, failure to thrive). GOR may be primary (due to anatomical or physiological abnormalities) or secondary to other diseases such as urinary tract infection, metabolic disorders, food allergy, etc (see table 1). The increasing availability of lower oesophageal pH monitoring, paediatric endoscopic skills, and newer more effective pharmacological agents has led to a greater awareness of the range of symptoms attributable to gastro-oesophageal reflux disease (GORD), and the need for a structured approach to diagnosis and management.

Diagnostic accuracy of pH monitoring in gastro-oesophageal reflux.

One hundred and eleven children admitted with suspected gastro-oesophageal reflux were studied, with 24 hour oesophageal pH monitoring as the first line of investigation. Barium swallow examination, or oesophagoscopy, or both, were carried out only in children with abnormal pH, who subsequently had a trial of 1-12 months medical treatment. All patients were followed up for eight months to two y...

Natural history of gastro-oesophageal reflux disease without oesophagitis (NERD)--a reappraisal 10 years on.

BACKGROUND AND AIM Ten years ago we published a study describing the 6-month outcome of 33 outpatients with typical gastro-oesophageal reflux disease symptoms and pH-metry proven excess gastro-oesophageal reflux but without endoscopical evidence of oesophagitis, currently referred to as patients with gastro-oesophageal reflux disease without oesophagitis. We now present an update of that report...

Omepr...
