Abnormalities of proximal femoral growth after severe Perthes' disease.

چکیده
We studied the pattern of proximal femoral growth after severe Perthes' disease (Catterall grade III or IV) by retrospective analysis of serial radiographs in 52 hips (46 patients). Our aim was to determine the relationship between proximal femoral growth abnormalities and metaphyseal cysts, epiphyseal extrusion, physeal narrowing, and extensive epiphyseal necrosis. The average follow-up after treatment was 9.8 years (range 4 to 16 years), and 37 of the hips were followed to skeletal maturity. Slowing of proximal femoral growth was common: symmetrical abnormality was seen in 26 hips and asymmetrical abnormality in nine. However, definite premature closure of the proximal femoral physis was seen in only three hips. Abnormality seemed to be due to altered growth velocity rather than to bar formation in most cases. Metaphyseal cysts, epiphyseal extrusion and physeal narrowing during the active stage of the disease, alone or in combination, were found to be neither sensitive nor specific predictors of the subsequent growth pattern.

Evolution in diagnosis and treatment of Legg-Calve-Perthes disease
Legg-Calvé-Perthes disease (LCPD) is an idiopathic osteonecrosis of the femoral head with variable complications and resultant deformity of the femoral head and osteoarthritis. Suggested risk factors are acetabular retroversion, obesity, latitude, hyperactivity, and coagulopathy. The most commonly applied classification is based on radiolucency in the lateral pillar of the femoral head, which i...
متن کاملRemarks on the etiology and pathogenesis of Perthes' disease: an experiment-based hypothesis.
The literature on Perthes' disease points up the significance of specific anatomical conditions affecting vascularization of the femoral head, as well as immaturity and mechanical weakening of the bone tissue in the etiology and pathogenesis of this disorder in children. An experimental study using calf femurs as models confirmed the author's hypothesis that the areas most susceptible to mechan...
متن کاملClinical Outcome of Femoral Osteotomy in Patients with Legg-Calve´-Perthes Disease
Background: Legg-Calve´-Perthes disease is a juvenile idiopathic osteonecrosis in which the blood supply of femoral head is not sufficient and the bone dies provisionally. The aim of this study is to evaluate outcome of Femoral osteotomy in children with LCPD in our University Hospital. Methods: In a descriptive analytic study, between 2008 and 2013, patients with the diagnosis of Legg-Calv...
متن کاملPerthes' disease and multiple epiphyseal dysplasia.
Five atypical cases were observed amongst ninety children with Perthes' disease, ten of whom had bilateral hip joint involvement. All five were boys, four being under 4 years of age. Four had bilateral hip joint disease, four presented with hip pain, three showing some degree of retardation of bone growth. In one case the hip disorder was familial, and in four there were bony abnormalities else...
متن کاملPerthes' disease of a severe type developing after satisfactory closed reduction of congenital dislocation of the hip. A report of three cases.
Some 2500 children with congenital dislocation of the hip were treated at the Orthopaedic Hospital of the Invalid Foundation over the period 1954 to 1974 and have been regularly followed up to the end of growth. One of us (Laurent 1953) had already observed that the vulnerability of the femoral head had often led to changes resembling Perthes’ disease. In 1957 preliminary traction and adductor ...
