Effect of gastrin heptadecapeptide ( G 17 ) on cesophageal contractions in patients with diffuse cesophageal spasm
نویسنده
چکیده
An intravenous bolus of pentagastrin significantly increased the amplitude and duration of cesophageal body contractions in seven patients with diffuse cesophageal spasm (DES) when compared with five normal subjects (p > 0-05). In order to determine whether this stimulation also occurred at physiological gastrin concentrations, the effect of an intravenous infusion of gastrin heptadecapeptide (G17), 25 pmol/kg-h, on cesophageal contractions was studied in DES patients. G17 had no significant effect on the amplitude and duration of cesophageal contractions compared with a saline control. This dose of G17 was near the D50 for gastric acid secretion and produced a rise in serum gastrin concentration comparable with a meal. G17 infusions at doses of 100 and 200 pmol/kg-h increased the amplitude and duration of cesophageal contractions, but the corresponding serum gastrin concentrations were higher than postprandial levels. Thus, endogenous fluctuations in serum gastrin heptadecapeptide, alone, are unlikely to alter cesophageal contractions in DES patients.

متن کاملRole of gastrin heptadecapeptide in the acid secretory response to amino acids in man.
Amino acids and peptides release gastrin and stimulate gastric acid secretion. However, the relation between gastrin release and acid secretory response is unclear. An isotonic mixed amino acid solution (casein hydrolysate) was continuously infused into the stomach of eight healthy human subjects. Acid secretion, measured by in vivo intragastric titration, increased 12.8 meq/h over the response...

The diagnosis of mitral incompetence, either alone or in association with mitral stenosis, often presents difficulties. In the present study an attempt has been made to provide a graphic method of confirming the diagnosis. Abnormal auricular filling consequent on reflux from the ventricle might be expected to produce abnormal movements of the contiguous portion of the cesophagus and to result i...

HAEMORRHAGE from cesophageal varices is a dreaded and frequently fatal complication of portal hypertension. In patients with cirrhosis the mortality within one year of the first hxmorrhage is from 25-75 per cent. The most effective way of preventing recurrent bleeding is reduction of the portal pressure by some form of portal-systemic shunt, but unfortunately there are patients where this is im...

The cesophagus lies close to the posterior surface ...
