چکیده
The EX-PRESS shunt (Alcon Laboratory) is a stainless steel implant designed to shunt aqueous from the anterior chamber to the subconjunctival space.Thedevice isplacedundera scleral flap, with the lumenideallypositionedparallel to the iris. A properly positioned EX-PRESS shunt creates acontrolledcommunicationbetween the anterior chamber and the bleb.1 Several shunt-specific complications havebeendescribed elsewhere, includingblockage, extrusionorexposure,andanteriordislocation.2-4 Inadditiontothese complications, malpositioning of the shunt can occur but, to our knowledge, has never been reported as a source of debilitating chronic eye pain requiring shunt removal. We describe a new technique for gonioscopy-assisted ab interno removal of an EX-PRESS shunt. We also report findings in a patient with severe eye pain due to indentation and chafing of the superior iris stroma caused by an iris-embedded shunt. The patient’s eye pain resolved completely after successful ab interno removal of the malpositioned shunt. In a second patient, the ab interno techniquewasused to removeanonfunctioning shuntduring another glaucoma procedure. Report of Cases

BACKGROUND We report a case of Ex-PRESS miniature glaucoma shunt obstruction resulting from progressive iris synechial formation after transient anterior chamber shallowing. CASE PRESENTATION A 68-year-old woman with pseudoexfoliation glaucoma in her right eye underwent filtration surgery with implantation of the Ex-PPESS shunt (model P-50, Alcon Japan, Tokyo, Japan) in combination with intra...

BACKGROUND To report a case of impending extrusion of Ex-PRESS shunt treated by shunt-position adjustment. CASE PRESENTATION A 56-year-old Asian woman presented with impending extrusion and malposition of Ex-PRESS shunt in her left eye. The bleb of the left eye was shallow and diffuse. In the past, the patient was treated by Ex-PRESS shunt implantation under the scleral flap in both eyes. The...
متن کاملSurgical treatment of neovascular glaucoma with Ex-PRESS glaucoma shunt
RATIONALE Neovascular glaucoma (NVG) is one of the most aggressive types of glaucoma, and its abnormal fibrovascular tissue growth on the iris and trabecular meshwork may create difficulties to control the intraocular pressure (IOP) and perform the operation such as trabeculectomy. PATIENT CONCERNS Ex-PRESS glaucoma shunt has been introduced to serve as one alternative operation for glaucoma,...
متن کاملStandard Trabeculectomy and Ex-PRESS Miniature Glaucoma Shunt: A Comparative Study and Literature Review
PURPOSE The aim of this study was to compare the efficacy and safety between standard trabeculectomy and the Ex-PRESS shunt implantation. METHODS A retrospective review of the records of 100 eyes of 100 patients who underwent trabeculectomy or Ex-PRESS shunt implantation between July 2010 and June 2012 was conducted. Of these, 61 (61%) eyes underwent trabeculectomy and 39 (39%) eyes underwent...
