Bronchoconstriction in response to deep inhalation during spirometry testing.

چکیده
In healthy individuals, airway resistance has an inverse relationship with lung volume.1 Moreover, deep inhalation is thought to maintain airway smooth muscle homeostasis and to possess bronchodilating and bronchoprotective properties against challenge agents.2-6 The bronchodilatory effect of deep inhalation is clearly impaired in asthma patients2,7,8; however, the bronchoprotective properties of deep inhalation against challenge test agonists (eg, methacholine) remain robust in many asthma patients with mild airway hyper-responsiveness.5,6 In some asthma patients, however, deep inhalation can produce paradoxical bronchoconstriction.5,9,10 This case describes a patient who developed significant bronchoconstriction in response to deep inhalation during spirometry testing.

Deep inhalation bronchodilation and oral corticosteroids in asthma.
A irway smooth-muscle hyperresponsiveness is a characteristic feature of asthma.1 Airway hyperresponsiveness is most commonly identified in the laboratory by the leftward shift of bronchoconstrictor (eg, methacholine) dose-response curves. A reduction in methacholine concentration producing a 20% fall in FEV1 indicates increased ease of development of bronchoconstriction.1–2 Subjects with asthm...
متن کاملPractical importance of a preceding full inhalation or exhalation upon the measurement of airway resistance.
1. Airway resistance was measured close to functional residual capacity before and after a full inhalation of total lung capacity, as well as before and after a full exhalation to residual volume. 2. The effects of these volume manoeuvres upon airway resistance (and associated lung volume) were determined in four resting normal male subjects and in six normal men during experimentally induced b...
متن کاملExpiratory reserve volume maneuver may be the preferred method for some patients during spirometry testing.
In the July 2012 issue of the Journal, Borg and Thompson1 reported the effect of 2 different vital capacity methods on lung volume data and patient classification derived from whole body plethysmography. The preferred method according to American Thoracic Society/European Respiratory Society recommendations is to follow closed-shutter panting with exhalation to residual volume (RV, expiratory r...
متن کاملOccupational asthma in workers of a pharmaceutical company processing spiramycin.
After investigation of one worker with occupational asthma, all 51 employees of a pharmaceutical company processing the macrolide antibiotic spiramycin were investigated to determine the frequency of the condition and the risk factors. The antibiotic was produced for short periods four to five times a year. The first part of the investigation, conducted before a production period, consisted of ...
متن کاملAssessing the effect of deep inhalation on airway calibre: a novel approach to lung function in bronchial asthma and COPD.
Bronchoconstriction in bronchial asthma and chronic obstructive pulmonary disease (COPD) may be due to decreased airway calibre and/or to the inability of the airways to distend after a deep inhalation (DI). The purpose of this review is to discuss the physiological and clinical relevance of this latter mechanism. During induced constriction, DI shows remarkable bronchodilatation in normal subj...
