Oblique Turnover Flap for Repositioning and Flattening of the Lateral Crura: A Novel Technique to Manage Cephalic Malposition of Lower Lateral Cartilage.

چکیده
Cephalic malposition of lower lateral cartilage (CMLLC) is one of the most common objects of aesthetic and functional rhinoplasty. Constantian wrote in his article that 68% of his primary rhinoplasty cases had lateral crural malpositioning.1 To diagnose CMLLC the surgeon has to observe several parameters that were described by Sheen: malposition is “any displacement of the lateral crura from the usual parallel alignmentwith the nostril rims,” that can be presented as abnormal axes of the lateral crura, bulbous, and broad nasal tip, parenthesis deformity, long alar creases, which extend to the nostril rims, alar wall hollows, alar retraction, nostril deformities, and incompetence of the external valve.2 Daniel measured the

The alar rim flap: a novel technique to manage malpositioned lateral crura.
BACKGROUND Alar cartilage malposition is a common anatomic variation in which the axis of the lateral crus lies cephalically and may be parallel to the cephalic septum. Malposition of the lateral crura may produce inward collapse of the alae that is observable on deep inspiration. OBJECTIVES The authors performed the alar rim flap technique to treat patients with alar malposition and assessed...

BACKGROUND Lower lateral crural deformities are common problems in rhinoplasty. The shape and position of the lower lateral crura directly influence the alar contour and external valve function. This study reviews an extensive experience with the lower lateral crural turnover flap, which represents a versatile and reproducible technique for correction of lower lateral crural deformities and imp...

BACKGROUND Cephalic malposition of the lower lateral cartilage (CMLLC) is a relatively common anatomical variant, particularly in Middle Eastern patients. The characteristics of CMLLC include long alar creases, a boxy and ball-shaped nasal tip, parenthesis tip deformity and external valvular incompetence. The gold standard for correcting CMLLC is the lateral crural strut graft (Gunter graft), b...
متن کاملLateral crural turn-in flap in functional rhinoplasty.
OBJECTIVE To use the trimmed cartilage as a support material for both internal and external valves. METHODS The lateral crural turn-in flap (LCTF) technique is simply to make cephalic trimming of the lateral crura and turn it into a pocket created under the remaining lateral crus. Twenty-four patients with lateral crura wider than 12 mm and in whom this technique was applied took part in this...

OBJECTIVE To apply a mathematical model to determine the relative effectiveness of various tip-plasty maneuvers while the lateral crura are in cephalic position compared with orthotopic position. METHODS A Matlab (MathWorks, Natick, Massachusetts) computer program, called the Tip-Plasty Simulator, was developed to model the medial and lateral crura of the tripod concept in order to estimate t...
