چکیده

Radiotherapy treatment planning requires a localization of the tumor within the patient. This is challenging to accomplish for microscopic infiltrative spread of disease that is not visible on current imaging modalities. Prime examples for infiltrative tumors are gliomas. With the help of mathematical models, common growth characteristics of gliomas, which are known from histopathological studies, can be incorporated in radiotherapy target delineation. This requires an imaging based personalization of the model to the individual patient. We demonstrate use cases of the Fisher-Kolmogorov glioma growth model in radiotherapy planning of a clinical case. We further analyze the crucial input parameters to the model, in particular, the need for reliable segmentation of anatomical boundaries such as the falx cerebri and the tentorium cerebelli.

Use of Diffusion Tensor Images in Glioma Growth Modeling for Radiotherapy Target Delineation

In radiotherapy of gliomas, a precise definition of the treatment volume is problematic, because current imaging modalities reveal only the central part of the tumor with a high cellular density, but fail to detect all regions of microscopic tumor cell spread in the adjacent brain parenchyma. Mathematical models can be used to integrate known growth characteristics of gliomas into the target de...

Radiotherapy planning for glioblastoma based on a tumor growth model: improving target volume delineation.

Glioblastoma differ from many other tumors in the sense that they grow infiltratively into the brain tissue instead of forming a solid tumor mass with a defined boundary. Only the part of the tumor with high tumor cell density can be localized through imaging directly. In contrast, brain tissue infiltrated by tumor cells at low density appears normal on current imaging modalities. In current cl...

Delineation of radiation therapy target volumes for patients with postoperative glioblastoma: a review

Glioblastoma is the most aggressive and lethal primary malignancy of the brain, and radiotherapy (RT) is a fundamental part of its treatment. However, the optimal radiation treatment conditions are still a matter of debate, and there is no clear consensus concerning the inclusion of peritumoral edema in the clinical target volume calculation. Target delineation calculations that use postoperati...

Optimization of clinical target volume delineation using magnetic resonance spectroscopic imaging (MRSI) in 3D conformal radiotherapy of prostate cancer

Background: For the purpose of individual clinical target volume assessment in radiotherapy of prostate cancer, MRSI was used as a molecular imaging modality with MRI and CT images. Materials and Methods: The images of 20 prostate cancer patients were used in this study. The MR and MRSI images were registered with CT ones using non-rigid registration technique. The CT based planning (BP), CT/MR...

Multimodal Analysis of Vasogenic Edema in Glioblastoma Patients for Radiotherapy Planning

Glioblastoma (GBM) is the most common type of primary brain tumor, which is characterized by an infiltrative growth pattern. In current practice, radiotherapy planning is primarily based upon T2 FLAIR MRI despite its known lack of specificity in the detection of tumor infiltration. While hyperintensity on T2 FLAIR is widely considered to represent infiltrative tumor, it may also be caused by th...
