Novel system for complete removal of secretions within the endotracheal tube: the Mucus Shaver.

چکیده
THE method normally used to clean endotracheal tubes (ETTs) in intubated patients is via a small, flexible, plastic suction catheter. The small-bore tube is inserted into the ETT and manipulated to remove mucus lodged within the ETT and this process is repeated as frequently as indicated. This may not remove all secretions. With residual secretions retained on the surface, the ETT provides a fertile ground for bacterial proliferation and biofilm formation. Bacteria-laden biofilm promotes bacterial colonization of the lower respiratory tract and development of ventilator-associated pneumonia. Moreover, any mucus accumulated within the ETT decreases its internal diameter, increasing resistance to airflow and work of breathing. To overcome this problem, we devised the Mucus Shaver (National Institutes of Health, Bethesda, Maryland).‡ In one sweep, the Mucus Shaver removes all mucus from the lumen of the ETT, restoring the appearance of the lumen to that of a new, unused ETT.

Endotracheal Tubes Cleaned With a Novel Mechanism for Secretion Removal: A Randomized Controlled Clinical Study.
INTRODUCTION Intubation compromises mucus clearance, allowing secretions to accumulate inside the endotracheal tube (ETT). The purpose of this trial was to evaluate a novel device for ETT cleaning. We hypothesized that its routine use would reduce tube occlusion due to mucus accumulation, while decreasing airway bacterial colonization. METHODS Subjects were randomized to either the use of the...

OBJECTIVE In patients intubated with endotracheal tubes (ETTs), suctioning is routinely performed to remove mucus from within the ETT and trachea. The Mucus Slurper is a novel ETT with built-in suction ports arranged radially at its tip. We evaluated the safety and efficacy of the Mucus Slurper, compared with conventional tracheal suction, to prevent airway obstruction in sheep with the ETT and...
متن کاملEfficacy Study of a Novel Device to Clean the Endotracheal Tube
The Endotracheal Tube (ETT) is a life-saving device for respiratory support and airway protection. However, its sustained presence in the trachea disrupts the physiological mechanisms that maintain mucus homeostasis. The process of mucus buildup and luminal narrowing leads to an increased ventilatory effort and might delay liberation from mechanical ventilation. In addition, potential lung path...

متن کاملVentilator associated pneumonia: evolving definitions and preventive strategies.
Ventilator-associated pneumonia (VAP) is one of the most frequent hospital-acquired infections occurring in intubated patients. Because VAP is associated with higher mortality, morbidity, and costs, there is a need to solicit further research for effective preventive measures. VAP has been proposed as an indicator of quality of care. Clinical diagnosis has been criticized to have poor accuracy ...
