The Jefferson Operative Room Setup for Endoscopic Skull Base Surgery
نویسندگان
چکیده
Introduction: As dedicated endoscopy suites for skull base surgery are becoming more prevalent, it is important to consider the optimal operative room setup. Multiple techniques have been presented for surgeon positioning for two-team endoscopic skull base surgery. Classically, the neurosurgeon and otolaryngologist stand adjacent to each other on the same side of the patient bed while operating. We describe an alternative approach utilizing dynamic endoscopy where the two surgeons stand across from each other. We discuss the merits of this approach from both a neurosurgical and otolaryngological perspective. Methods and results: We describe our technique of surgeon positioning, monitor positioning, operating room setup, and their implications on ergonomics and surgeon fatigability. Conclusion: We believe our approach of surgeon positioning, where surgeons stand on opposite sides of the patient bed while operating, to be a viable and more ergonomic alternative to the classical approach.

Sellar reconstruction algorithm in endoscopic transsphenoidal pituitary surgery: experience with 240 cases
Background: Proposing a strategy for sellar reconstruction in endoscopic transsphenoidal transsellar approach for pituitary adenoma. Methods: 240 patients with pituitary adenoma underwent pure endoscopic endonasal transsphenoidal surgery. Intra-operative CSF leaks were classified as grade 0, no observable leak grade 1, CSF dripping through an arachnoid membrane defect of less than 1 ...
متن کاملIncidence and management of epistaxis after endoscopic skull base surgery.
BACKGROUND/AIMS Endonasal endoscopic surgery continues to gain acceptance as a minimally invasive, effective approach for benign and malignant pathology of the paranasal sinuses and skull base. Postoperative epistaxis could potentially result in rapid and devastating consequences due to hemorrhage. Our objective was to assess the incidence and causes of epistaxis after endoscopic skull base sur...

Abstract Background: Over the past 20 years, the minimally invasive endoscopic approach has gained widespread acceptance. The study was performed to evaluate the diagnostic method and the success rate of endoscopically diagnosed and treated CSF rhinorrhea, and also investigations such as leakage site and etiologic factor. Methods: This retrospective CSF leakage management review of ...
متن کاملManagement of Carotid Artery Injury in Endonasal Surgery
Introduction Carotid artery injury (CAI) is the most feared and potentially catastrophic intraoperative complication an endoscopic skull base surgeon may face. With the advancement of transnasal endoscopic surgery and the willingness to tackle more diverse pathology, evidence-based management of this life-threatening complication is paramount for patient safety and surgeon confidence. Objective...
