Diagnosis of a postsurgical aortic pseudoaneurysm in Shone syndrome by cardiovascular magnetic resonance.

چکیده
A 34-year-old male was admitted with increasing dyspnea on exertion for the past 3 years. He was born with shone syndrome. Shone syndrome was first described by Shone et al. in 1963 and consists of four obstructive, or potentially obstructive conditions of the heart. These are a “parachute” deformity of the mitral valve, supravalvular ring of the left atrium, subaortic stenosis, and aortic coarctation. He had repair of a coarctation of aorta within the first year of his life. At 7, he underwent mitral and aortic valvuloplasty and at 15 had aortic valvulotomy and aortoplasty. Five years later, he underwent repair of an ascending aortic aneurysm with a Dacron graft, reimplantation of the coronary ostia by a button technique, and valve replacements with a Bjork–Shiley mitral valve prosthesis and St. Jude aortic valve prosthesis. He was asymptomatic until age 31 when he was admitted with hemoptysis but discharged after negative bronchoscopy and V/Q scan. He continued to become progressively more dyspneic on exertion. He underwent right and left heart catheterization and transthoracic echocardiography, which showed pulmonary hypertension (Mean PA 1⁄4 46 mm Hg), increased right ventricular pressure and the presence of an aortic pseudoaneurysm (PSA) extending from the proximal ascending aorta and compressing the left atrium. On angiography it was noted that the left circumflex originated from the right coronary artery but it was not possible to visualize left main or left anterior descending artery. Cardiovascular MR (CMR) was performed including MR angiography with gadolinium DTPA. This showed the previously placed tube graft in the ascending aorta connected to a large PSA

Pseudoaneurysm of the mitral–aortic intervalvular fibrosa presenting after chest trauma and diagnosed by cardiac magnetic resonance: a case report
UNLABELLED INTRODUCTION Annular subvalvular pseudoaneurysm is a rare example of left ventricle aneurysm described predominantly in young African people. These aneurysms are divided into two different types, namely, submitral or subaortic, with subaortic being the less frequent kind. The subaortic type is most often localized in the mitral-aortic intervalvular fibrosa. To the best of our know...
متن کاملLate lumen loss in thoracic aortic end graft after endovascular procedure of a traumatic pseudoaneurysm
We report the case of a 23-year-old woman who died due to endograft stenosis 20 months after thoracic endovascular aortic repair. The patient presented with the pseudocoarctation syndrome. Although angioplasty of stenosis endograft was successfully performed, severe metabolic complications were lethal.
متن کاملCase of Delayed Diagnosis of Shone Syndrome in a Patient Planning for Pregnancy
Introduction: Shone syndrome is a rare congenital cardiac abnormality; however, many of the cases remain undiagnosed until early and mid-adulthood. Different Imaging modalities are used to assess the related structural abnormalities. Case presentation: In this case study, we report a 32 yo woman who was planning for her first pregnancy. In consideration to her childhood heart problems, besides...
متن کاملMycotic pseudoaneurysm associated with aortic coarctation.
Development of a mycotic aneurysm or pseudoaneurysm after subacute bacterial endarteritis is uncommon. Nonetheless, patients with coarctation of the aorta are more likely to develop this complication. We describe a case of a large pseudoaneurysm discovered in a child with a previously undiagnosed aorta coarctation. Successful repair was performed with the aid of partial left heart bypass and th...
متن کاملBlood Flow Simulation in an Aorta with a mild coarctation Using Magnetic Resonance Angiography and Finite Volume Method
Coarctation of the aorta is one of the five main congenital cardiovascular failures, accounting for 6–8 percent of these failures. This research aimed to simulate the blood flow of a seventeen-year-old male teen with a mild coarctation at one-third of his aorta's descending branch. The simulation was performed by extracting the domain and the input pulsatile velocity signal as the boundary cond...
