Statin therapy: beyond cholesterol lowering and antiinflammatory effects.

چکیده
Aggressive cholesterol lowering has gradually been adopted by clinicians as part of a standard treatment regimen for patients with documented coronary artery disease, including those who have had recent bypass surgery and acute coronary syndrome.1–3 According to the recently published National Cholesterol Education Program Adult Treatment Panel III (NCEP III) guidelines, those patients who have documented coronary disease should have their LDL cholesterol lowered to less than 100 mg/dL.

Pleiotropic effects of statins
The lipid-lowering actions of statins are well known. However, recent studies provide compelling evidence that the clinical benefits of statin therapy may also be attributed to mechanisms independent of their cholesterol-lowering effects. These non-lipid-lowering (pleiotropic) effects of statin therapy are believed to include antiinflammatory actions, property to reverse endothelial dysfunction...
متن کاملOptimal Lipid Modification: The Rationale for Combination Therapy
BACKGROUND An emphasis on more aggressive lipid-lowering, particularly of low-density lipoprotein cholesterol, to improve patient outcomes has led to an increased use of combination lipid-lowering drugs. This strategy, while potentially beneficial, has triggered concerns regarding fears of adverse effects, harmful drug interactions, and patient nonadherence. OBJECTIVE To present key data rega...
متن کاملA review of high-dose statin therapy: targeting cholesterol and inflammation in atherosclerosis.
Lipid lowering with 3-hydroxy-3-methylglutaryl coenzyme A reductase inhibitors or 'statins' has dramatically reduced morbidity and mortality in patients with established cardiovascular disease. Recently, there have been multiple studies investigating the role of high-dose statin therapy with more aggressive lipid lowering in this setting. Concomitantly, there is increasing evidence implicating ...
متن کاملBeyond the laboratory: clinical implications for statin pleiotropy.
Results from large-scale clinical trials of lipid lowering with 3-hydroxy-3-methylglutaryl-coenzyme A (HMG-CoA) reductase inhibitors (statins) have led to a revolution in the management of atherosclerosis. In addition to their potent effect on serum lipid levels, statins influence several other cellular pathways, including those involving inflammatory, oxidative, and thrombotic processes. These...
متن کاملOvercoming toxicity and side-effects of lipid-lowering therapies.
Lowering serum lipid levels is part of the foundation of treating and preventing clinically significant cardiovascular disease. Recently, the American Heart Association/American College of Cardiology released cholesterol guidelines which advocate for high efficacy statins rather than LDL-c goals for five patient subgroups at high risk for cardiovascular disease. Therefore, it is critical that c...
