Coagulopathy in Critically Ill Patients

چکیده
www.chestjournal.org CHEST / 137 / 1 / JANUARY, 2010 185 C about the risk of bleeding in critically ill patients are common. This apprehension may stem from a sense of responsibility for bleeding following procedures or anticoagulant therapy, or perhaps the fact that hemorrhage is often visible externa lly. In addition, the clinical laboratory may raise awareness by offering tests that clinicians think provide information about impaired clotting. Unfortunately, many intensivists are not skilled or trained in carrying out assessments of coagulation testing or disorders. As the second of a two-part series on coagulopathy in critically ill patients, this article reviews soluble clotting factors and the most frequent coagulation disorders seen among adults in the ICU. Although this article does not discuss thrombophilia in detail, it does provide a framework for understanding hemostatic testing and interpreting common coagulation tests. Background: The application of more humanized approaches in the caring process of the Intensive Care Unit (ICU) patients with the aim of improving the quality of care has been given a growing attention However, there are limited studies regarding the Islam-based caring for the Moslem populations in Indonesia. Aim: This study aimed to explore and describe the caring actions employed by nurses to...
متن کاملUsing Iron-Chelating Agents in Critically Ill Patients with Iron Overload. Fact or Fiction?
Recently, some evidence has shown that the failure of iron homeostasis may occur in critically ill patients and can lead to iron overload. Elevated ferritin levels as a body iron burden index in critically ill patients may be associated with depressed level of consciousness and greater mortality. However, the necessity of using iron-chelating agents in clinical situation is still unknown for th...

Background & Objective: Vitamin K deficiency is one of the commonest causes of coagulopathy and bleeding in patients on prolonged antibiotic therapy. This study was conducted to evaluate the status of vitamin K deficiency in hospitalized children on prolonged antibiotic therapy and its role in reversing the coagulopathy. Method: A prospective non-randomized, cross-sectional study was conduc...
متن کاملPharmacokinetic Behavior of Theophylline following PEEP in Critically Ill Patients with Acute Lung Injury
The effect of Positive End Expiratory Pressure (PEEP) on the hepatic elimination of low to moderate extraction ratio drugs has not been clearly defined. We prospectively investigated the effect of PEEP on the clearance of theophylline in 30 (20 males and 10 females) intubated critically ill adult patients with acute lung injury/acute respiratory distress syndrome (ALI/ARDS). The Mean (±SD) age ...
متن کاملHemostasis and endothelial damage during sepsis.
The sepsis syndrome represents a disease continuum, including severe sepsis and septic shock associated with high mortality. One of the main problems in severe sepsis and septic shock, resulting in organ failure and death, are disturbances in the hemostasis due to sepsis-related coagulopathy. Sepsis-related coagulopathy affects not only traditional coagulation factors, but also the platelets an...
متن کاملRisk estimation and management of coagulopathy in ICU patients in need of an invasive procedure
Coagulation abnormalities have a high prevalence in critically ill patients. The most commonly used tests to assess coagulation status include assessment of the platelet count, prothrombin time, fibrinogen and d-dimer. However, as these tests do not reflect in vivo haemostatic potential, their ability to assess a potential risk of bleeding is limited. Viscoelastic tests evaluate the whole proce...
