Adipocyte-derived hormone levels in HIV lipodystrophy.

چکیده
OBJECTIVE Despite evidence for the role of adipocyte-derived hormones in insulin resistance, little is known about their levels in human lipodystrophic states. We examined the relationships of plasma leptin and adiponectin levels to fat distribution and insulin sensitivity in the HIV lipodystrophy syndrome. DESIGN Cross-sectional study SETTING HIV primary care practices PATIENTS HIV-infected men with (n=13) and without (12) lipodystrophy and healthy uninfected controls (12). MAIN OUTCOME MEASURES Plasma adiponectin and leptin levels were measured in the fasting state. Body composition was assessed by physical examination, dual-energy x-ray absorptiometry and computed tomography. Insulin sensitivity (S(I)) was measured using the insulin-modified frequently sampled intravenous glucose tolerance test. RESULTS Leptin levels were significantly higher in HIV-infected men with lipodystrophy as compared to HIV-infected controls (5.2 vs 3.0 ng/ml, P=0.01). Across the entire study population, leptin levels were positively correlated with measures of general adiposity. In the HIV-infected patients, leptin levels were negatively correlated with S(I) after adjustment for fat mass (r=-0.38, P=0.07). Adiponectin levels were significantly lower in HIV-infected men with lipodystrophy as compared to both HIV-infected and healthy controls (1.6 vs 3.4 microg/ml, P<0.05 and 1.6 vs 6.7 microg/ml, P<0.001, respectively). Adiponectin levels, after adjustment for fat mass, were correlated with measures of fat distribution. Finally, in the HIV-infected patients, adiponectin levels were significantly and positively correlated with S(I) after adjustment for fat mass (r=0.75, P < or = 0.001), and adiponectin level was also an independent determinant of S(I). CONCLUSIONS Plasma leptin and adiponectin levels are altered in the HIV lipodystrophy syndrome. Adiponectin deficiency may play a role in the insulin resistance associated with HIV lipodystrophy.

Regulation of adiponectin in adipocytes upon exposure to HIV-1.
OBJECTIVES Adipose dysregulation, dyslipidemia, and insulin resistance are hallmarks of HIV-related lipodystrophy. The precise mechanisms behind these disturbances are unknown. In HIV-infected patients, we previously demonstrated a strong relationship between lipodystrophy and levels of adiponectin, an adipose peptide implicated in regulation of glucose and lipid metabolisms. In this study we i...
متن کاملLeptin in Relation to the Lipodystrophy-Associated Metabolic Syndrome
Leptin, an adipocyte-secreted hormone, regulates energy homeostasis as well as reproductive, neuroendocrine, immune and metabolic functions. Subjects with decreased amounts of fat in their adipose tissue, i.e., lipoatrophy, have low leptin levels. In the context of open-label, uncontrolled studies leptin administration, in physiological replacement doses, has been shown to have metabolically sa...
متن کاملAdipokines, hormones related to body composition, and insulin resistance in HIV fat redistribution syndrome
BACKGROUND Lipodystrophies are characterized by adipose tissue redistribution, insulin resistance (IR) and metabolic complications. Adipokines and hormones related to body composition may play an important role linking these alterations. Our aim was to evaluate adipocyte-derived hormones (adiponectin, leptin, resistin, TNF-α, PAI-1) and ghrelin plasma levels and their relationship with IR in HI...
متن کاملProtease inhibitor treatments reveal specific involvement of matrix metalloproteinase-9 in human adipocyte differentiation.
We previously showed that human and murine 3T3-F442A preadipocytes produced and released matrix metalloproteinases (MMPs) 2 and 9 and that a treatment by MMP inhibitors resulted in the blockade of murine fat cell adipose conversion. In parallel, investigators reported that other protease inhibitors, the human immunodeficiency virus (HIV) protease inhibitors (PIs) involved in lipodystrophy in hu...
متن کاملReport from the 10th International Workshop on Adverse Drug Reactions and Lipodystrophy in HIV.
OBJECTIVES: The use of thymidine NRTI (tNRTI) drugs for treatment of HIV, specifically stavudine and zidovudine, increases the risk of developing lipoatrophy – a pathology of the adipose tissue involving reduction in mitochondrial (mt) DNA copy numbers per adipocyte, tissue fibrosis and increased adipocyte death. Adipose tissue pathology in other settings represents a significant risk for cardi...
