چکیده

Exocrine pancreatic insuffiency is loss of ability to properly digest food due to inadequate pancreatic enzyme activity. The etiology of exocrine pancreatic insuffiency, in humans, is mostly chronic pancreatitis or diseases affecting pancreas such as cytsic fibrosis, Shwachman-Diamond syndrome. It may also occur as a complication of gastric or pancreatic surgical procedures due to decrease of exocrine enzyme producing cell reserve. Symptoms related with exocrine pancreatic insuffiency are associated with maldigestion including steatorhea, diarrhea, and abdominal pain and weight loss. Diagnosis can be made on clinical observation that is supported with direct and indirect tests for exocrine pancreatic insuffiency. So called, 13C labeled mixed triglyceride breath test offer promising results. Supplemental pancreatic-enzyme replacement therapy is widely accepted method of choice or treatment of exocrine pancreatic insuffiency. Main goal of pancreatic-enzyme replacement therapy is to minimize malabsorption and associated malnutrition especially of lipids and lipid soluble vitamins. There are various preparations of pancreatic enzyme formulas according to ingredients and physical properties such as Kreon (Abbott, IL,USA), Zenpep (Eurand, Milan, Italy) and Pancreaze (Johnson & Johnson, NJ, USA). Exocrine pancreatic insufficiency is not an infrequent clinical entity and replacement therapy can be initiated for proven cases or empirically for patients with mild symptoms. Enteric-coated preparations should be given with recommended dosage.

Exocrine Pancreatic Insufficiency and Diabetes Mellitus

Gastrointestinal symptoms are common in patients with diabetes mellitus. One possible cause for these symptoms is exocrine pancreatic insufficiency, and although several hypotheses have been proposed to explain the aetiology, the precise pathophysiological mechanisms remain to be elucidated. The prevalence and the clinical importance of exocrine pancreatic insufficiency are debatable. Exocrine ...

Fluorescein dilaurate test of exocrine pancreatic function in cystic fibrosis.

The fluorescein dilaurate test, a non-invasive test of exocrine pancreatic function, was carried out on 21 children with cystic fibrosis and pancreatic exocrine insufficiency, and 12 healthy siblings. The test clearly discriminated between the patients with cystic fibrosis and severe exocrine pancreatic insufficiency and the normal control subjects.

Pancreatic polypeptide and exocrine pancreatic function in the elderly.

The relationship between exocrine pancreatic function and plasma pancreatic polypeptide levels was studied in 14 normal elderly subjects and in ten elderly patients with exocrine pancreatic insufficiency determined by the para-amino-benzoic acid test. There was a decrease in the total pancreatic polypeptide response after a standard mixed meal in the group with pancreatic insufficiency (t = 2.7...

Pancreatic fibrosis correlates with exocrine pancreatic insufficiency after pancreatoduodenectomy.

BACKGROUND Obstruction of the pancreatic duct can lead to pancreatic fibrosis. We investigated the correlation between the extent of pancreatic fibrosis and the postoperative exocrine and endocrine pancreatic function. METHODS Fifty-five patients who were treated for pancreatic and periampullary carcinoma and 19 patients with chronic pancreatitis were evaluated. Exocrine pancreatic function w...

Pancreatic Endocrine and Exocrine Function in Children following Near-Total Pancreatectomy for Diffuse Congenital Hyperinsulinism

CONTEXT Congenital hyperinsulinism (CHI), the commonest cause of persistent hypoglycaemia, has two main histological subtypes: diffuse and focal. Diffuse CHI, if medically unresponsive, is managed with near-total pancreatectomy. Post-pancreatectomy, in addition to persistent hypoglycaemia, there is a very high risk of diabetes mellitus and pancreatic exocrine insufficiency. SETTING Internatio...

Treatment of Pancreatic Exocrine Insufficiency with Enteric Coated Pancreatin Formulations: An Overview

Pancreatin is a mixture of several digestive enzymes produced by the exocrine cells of the pancreas. It is composed of amylase, lipase and protease. It is used to treat conditions in which pancreatic secretions are deficient, such as surgical pancreatectomy, pancreatitis and cystic fibrosis. Pancreatin products contain the pancreatic enzymes trypsin, amylase and lipase. The patients with pancre...
