چکیده

Atlanto-axial dislocation IS an uncommon condition. It is usually secondary to trauma, infection (tuberculous and non-tuberculous atlas and axis) and rheumatoid arthritis. Other uncommon causes reported include retropharyngeal abscess, malignant deposits, ankylosing spondylitis (Sorin, et al, 1979). Congenital atlanto-axial dislocation is rare. Wadia (1967) first drew attention to its relative common occurrence among his patients in India. This finding has since then been substantiated by other authors (Bharucha et al., 1964). Congenital atlanto-axial dislocation is also said to be relatively common in Ceylon and Thailand (Wadia, 1972). Rheumatoid arthritis is not as commonly found in Malaysia as in the western developing countries. Its pattern of illness seen here also tends to be of milder form of illness (Toh, et al., 1973). Ethnically Indians constitute about ten percent of the multiracial population of Malaysia. It is thus interesting to note that since the inception of the University Hospital in 1968, we have seen three cases of cong-enital atlanto-axial

Occipito-cervical fusion. Indications, technique and results.

Fusion ofthe occiput to the upper cervical spine is an uncommon but important operation as it is the only means ofpermanently stabilising the interveningjoints. Instability ofthe atlas or axis on the remainder of the cervical spine is dangerous, and if it progresses to subluxation or dislocation there is a risk of damage to the medulla, spinal cord and nerve roots. Fusion may be required for pr...

Atlanto-axial rotatory fixation and fracture of the clavicle. An association and a classification.

Five children with atlanto-axial rotatory fixation (AARF) in association with fractures of the clavicle are described. It is postulated that the rotary fixation is a direct result of the trauma which produces the fracture. The importance of early diagnosis is stressed, since delayed diagnosis may lead to chronic deformity. Early diagnosis depends on awareness of the possibility of AARF, and eit...

Management of congenital atlanto-axial dislocation: some lessons learnt.

Congenital atlantoaxial dislocation (AAD) has a high incidence in India. In these patients, even a minor trauma may precipitate severe morbidity. The management of mobile AAD consists of posterior stabilization. In fixed AAD, the offending compressive element is present anterior to the cervicomedullary junction, which should be generously removed by the transoral approach. In many of these pati...

The Transoral Approach to Clivus and Cervical Spine: Resurrection of a Technique Fading in Time

The transoral approach to clivus and upper cervical region is a direct route for treating pathology ventral to the brainstem and upper cervical spinal cord with low morbidity and mortality rates. This approach may be used for congenital atlanto-axial dislocation, fracture dislocation, vertebrobasilar aneurysms, basilar invagination, compressive rheumatoid pannus and tumours of the craniovertebr...

Rotatory dislocation of both atlanto-axial joints.

A case of rotatory dislocation of both atlanto-axial joints is presented. A review of the literature reveals two other cases, both in children. This would appear to be the first report in an adult, who survived and made a full recovery.
