چکیده

Coronary artery disease, ischemic cardiomyopathy, and congestive heart failure continue to confer significant morbidity and mortality despite advances in medical therapy, percutaneous coronary interventions, and surgical revascularization.1,2 This unmet clinical need is the rationale for novel therapeutic strategies like therapeutic angiogenesis. Unlike traditional revascularization, which focuses on macrovascular solutions, angiogenesis seeks to improve the microcirculation by stimulating new capillary and collateral arterial vessel formation. Recently, there has been significant discussion on traditional revascularization, in particular the benefit of revascularization with coronary artery bypass surgery (CABG) in patients with left ventricular (LV) dysfunction in the Surgical Treatment for Ischemic Heart Failure (STICH) Trial.3 As opposed to mechanical revascularization, whether by CABG or percutaneous coronary intervention, therapeutic angiogenesis has the advantage of improving coronary microcirculation by harnessing existing physiological processes to address a biological problem.

Time course of functional recovery of stunned and hibernating segments after surgical revascularization.

BACKGROUND Recovery of function is possible in patients with ischemic cardiomyopathy when left ventricular dysfunction is caused by stunning or hibernation. It is plausible that recovery of function after revascularization may take a longer time in hibernating myocardium compared with stunned myocardium. Accordingly, the time courses of functional recovery in hibernating and stunned myocardium ...

Dobutamine stress echocardiography identifies hibernating myocardium and predicts recovery of left ventricular function after coronary revascularization.

BACKGROUND The identification of hibernating myocardium is important in selecting patients who will benefit from coronary revascularization. This study was performed to determine whether dobutamine stress echocardiography (DSE) could identify hibernating myocardium and predict improvement in regional systolic wall thickening after revascularization. METHODS AND RESULTS DSE was performed in 49...

The use of nuclear imaging to assess the myocardial viability.

Terms commonly used to describe different myocardial states are ‘‘viable’’, ‘‘stunned’’, ‘‘hibernating’’, ‘‘infarcted’’, and ‘‘scarred’’. Common problems are to equate infarcted myocardium with irrecoverable dysfunction, making no allowance for partial-thickness infarction, and to equate viable myocardium with hibernation. It is particularly important to distinguish between viable and hibernati...

Efficacy of coronary angioplasty for the treatment of hibernating myocardium.

OBJECTIVES To determine the efficacy of coronary angioplasty as the sole method of revascularisation in patients with coronary artery disease and chronically dysfunctional but viable myocardium (hibernating myocardium), and to assess the effect of restenosis on functional outcome. DESIGN AND PATIENTS 24 consecutive patients with hibernating myocardium were studied. Positron emission tomograph...
