Due to the low clinical efficacy of current antiarrhythmic agents, various nonpharmacologic approaches including catheter ablation, permanent cardiac pacing and surgical procedure alone or in combination, have been developed for treatment of AF. Except in patients who require open heart surgery, surgical procedures have very limited role in the treatment of AF. Pacing therapy alone, except in patients with concomitant bradycardia, has a very limited role for the treatment of AF. The incremental benefit of alternate or multiple site pacing and different pacing algorithms remains to be defined. Catheter ablation procedures may prove to provide curative therapy for AF in selected but widening patient populations. However, issues concerning the optimal ablation technique and safety of the procedure remain to be resolved. Currently, most of the nonpharmacologic methods of AF treatment are more or less experimental and need to be proven by randomized control before they can be considered as routine therapy for AF.

Approaches to restoring and maintaining normal sinus rhythm.
The primary strategies in managing patients with atrial fibrillation (AF) are rate control, termination of the arrhythmia, and prevention of recurrences and thromboembolic events. Although there have been great advances in nonpharmacologic therapies to achieve these aims, drug therapy remains firstline treatment for patients with AF. This review briefly profiles the drugs most commonly used for...

The mainstay of treatment for atrial fibrillation, AF, remains pharmacologic control, either by maintaining sinus rhythm or by controlling the ventricular rate and allowing AF to continue. In patients where pharmacologic therapy i not effective, not tolerated or contraindicated, nonpharmacologic treatment may be beneficial. In the last two decades the number of nonpharmacologic treatment option...
متن کاملAtrial fibrillation in heart failure.
Heart failure (HF) and atrial fibrillation (AF) are highly prevalent debilitating conditions that often coexist and are frequently encountered in clinical practice. The presence of chronic AF is a marker of worse prognosis in patients with HF, and the onset of new AF in those with chronic HF is associated with increased morbidity and mortality. Advances in the development of novel drugs, nonpha...

Atrial fibrillation (AF) is associated with inflammatory and hypercoagulability state. Previous studies evaluated the safety and efficacy of dabigatran and warfarin in prevention of thrombothic complications. This study was intended to assess the influence of these drugs on hemostatic and inflammatory markers among patient underwent pulmonary vein ablation. A total of 100 patients with AF w...
