Exenatide: a novel treatment of Type 2 diabetes

A problem in developing glucagon-like peptide-1 as a therapeutic compound is that the hormone is rapidly inactivated by dipeptidyl peptidase-4. To overcome this, long-acting dipeptidyl peptidase-4, resistant receptor agonists have been explored. One such promising agonist is exenatide, which was isolated as exendin-4 from the lizard Gila monster. Exenatide, administered twice daily subcutaneously, improves metabolic control in subjects with Type 2 diabetes, both when given alone and together with other treatments. The durability is good – a 30-week study showed that HbA1c was reduced by 1.0%. This observation is also seen in association with weight loss. Furthermore, clinical studies have shown that exenatide is safe and tolerable. Mild-to-moderate nausea and vomiting are the most consistent adverse events – it is seen in the early phases of treatment in approximately 30% of subjects given exenatide. The experience so far shows that exenatide offers a promising novel therapy for Type 2 diabetes, based on an interaction with glucagon-like peptide-1 receptors.

Exenatide (Byetta) as a novel treatment option for type 2 diabetes mellitus.
Exenatide is the first drug in the incretin mimetic class and is indicated for treatment of type 2 diabetes mellitus. Although structurally similar to the native glucagon-like peptide, this synthetic form has a much longer duration of action. Randomized trials have shown exenatide to be efficacious in improving glycemic control when combined with either metformin or a sulfonylurea. The dose is ...
متن کاملEffects of Exenatide in a Morbidly Obese Patient with Type 2 Diabetes
INTRODUCTION The effect of exenatide in weight loss has been reported. Presented here is a case of a morbidly obese patient with type 2 diabetes using exenatide who dramatically lost her body weight in a year and experienced improved glycemic control. CASE REPORT Exenatide therapy was initiated for a 59-year-old morbidly obese Japanese woman with type 2 diabetes. To examine the effects of the...
متن کاملCost-effectiveness of intermediate or long-acting insulin versus Exenatide in type 2 diabetes mellitus patients not optimally controlled on dual oral diabetes medications
OBJECTIVE To better understand exenatide's role in the treatment of type 2 diabetes, this analysis assessed its cost-effectiveness in comparison to an intermediate (NPH) and long-acting insulin (glargine). Exenatide is a recently approved medication for the treatment of type 2 diabetes for use in addition to frequently used oral diabetes medications. METHODS Two studies were identified by a M...
متن کاملMedication utilization patterns among type 2 diabetes patients initiating Exenatide BID or insulin glargine: a retrospective database study
BACKGROUND Type 2 diabetes is a common and costly illness, associated with significant morbidity and mortality. Despite this, there is relatively little information on the 'real-world' medication utilization patterns for patients with type 2 diabetes initiating exenatide BID or glargine. The objective of this study was to evaluate the 'real-world' medication utilization patterns in patients wit...

OBJECTIVE Exenatide treatment improves glycemia in adults with type 2 diabetes and has been shown to reduce postprandial hyperglycemia in adolescents with type 1 diabetes. We studied the effects of exenatide on glucose homeostasis in adults with long-standing type 1 diabetes. RESEARCH DESIGN AND METHODS Fourteen patients with type 1 diabetes participated in a crossover study of 6 months' dura...
