Comparative effectiveness research: do employers care? What will they do? What about productivity outcomes?

چکیده
Recent Federal legislation, including the Patient Protection and Affordable Care Act (ACA), has launched a comprehensive national effort to conduct and disseminate comparative effectiveness research (CER). As purchasers of health care for employees, dependents and retirees, employers stand to benefit from potential quality and cost impacts of CER. Further, because they can influence patients, providers and provider organizations through health programs and benefit design, employers have the ability to contribute to the translation of CER findings into clinical practice, enabling quality and cost improvements. Our research indicates that a sizable segment of employers (specifically, large and self-insured employers who comprised our sample) are aware of CER and expect that CER findings will help them achieve health improvement and cost management goals. These employers have clear ideas about how they would use health programs and benefit strategies to help translate CER findings into practice. Our research further indicates that employers would value CER outcomes related to workforce productivity, and believe it is important for CER research to consider the impact of alternative treatments on absence, disability and work performance when appropriate. The Patient-Centered Outcomes Research Institute (PCORI) is by far employers’ most trusted source of CER information; and while some employers plan to directly monitor CER developments, most will rely on their health plans, consultants and Pharmacy Benefit Managers (PBMs) to keep them apprised of relevant matters and to help identify and implement effective program and policy actions. Employers are important health care stakeholders, and–particularly if findings address their interests in workforce productivity as well as employee health and health care costs–employers will be an ally in the effort to translate CER findings into better clinical practice, improved health, enhanced productivity and lower costs. 1

The Patient-centered Outcomes Research Institute’s Role in Advancing Methods for Patient-centered Outcomes Research
Currently, there is wide-spread recognition that much of health care is insufficiently grounded in evidence as to what works for whom under what circumstances. An important reason for this is that research efforts often do not compare alternative options of health care nor are they sufficiently focused on outcomes that matter to patients. In addition, the methods to conduct research designed to...

The development and evolution of any system–person, organization–nation depends on how the system succeeds to bridge the gap between what the system knows and what the system does (with the knowledge). We call this the gap between knowing and doing or the knowing-doing gap. If the system does not do what it knows, it will lose out in competition with other systems, its relative performance in...
متن کاملIt Ain’t What You Do (But the Way That You Do It): Will Safety II Transform the Way We Do Patient Safety; Comment on “False Dawns and New Horizons in Patient Safety Research and Practice”
Mannion and Braithwaite outline a new paradigm for studying and improving patient safety – Safety II. In this response, I argue that Safety I should not be dismissed simply because the safety management strategies that are developed and enacted in the name of Safety I are not always true to the original philosophy of ‘systems thinking.’
متن کاملEvaluation of an evaluation
Introduction. Evaluation is a systematic way to improve and make more effective actions that involves procedures which are useful, feasible, ethical, and accurate. Common questions in all evaluations are: do all part of program do well and effective? What is the good functioning? Why the program or its parts do not work well? What are the effects and consequences of the program? Is this progra...
متن کاملComparative effectiveness research: what to do when experts disagree about risks
BACKGROUND Ethical issues related to comparative effectiveness research, or research that compares existing standards of care, have recently received considerable attention. In this paper we focus on how Ethics Review Committees (ERCs) should evaluate the risks of comparative effectiveness research. MAIN TEXT We discuss what has been a prominent focus in the debate about comparative effective...
