Spinal pseudoarthrosis: a complication of ankylosing spondylitis.

چکیده
Fracture of the cervical spine is a recognized complication of advanced ankylosing spondylitis ( 1 ). Such fractures occur after relatively minor trauma and may appear t o be spontaneous. They are characteristically seen long after the cervical apophyseal joints have fused and vertebral osteoporosis has developed. This complication is consistent with the mechanically unstable situation that develops late in the disease: the heavy weight of the head is seated on an atrophic column of bone with numerous weak areas represented by the fused apophyseal joints and atrophic intervertebral discs. Such fractures, usually attended by neurologic complications, are often fatal. It is not widely appreciated that in this disease fractures may also occur in the lower spine. Unlike the cervical fractures, these are often clinically unrecognized but may cause pseudoarthrosis of a discovertebral joint which results in extensive bone resorption. Such lesions may have neurologic consequences, depending on the level of involvement. The following case illustrates the early and late roentgen features of this complication and documents its progressive nature over a 5-year period.

Surgical treatment of spinal pseudoarthrosis in ankylosing spondylitis.
BACKGROUND In ankylosing spondylitis (AS), the spine with osteoporotic changes is fragile and vulnerable to trauma. Patients may develop spinal pseudoarthrosis (SP), whose clinical features are usually misdiagnosed as a tuberculous infection. This study reports our experience with surgical treatment for SP in AS. METHODS Eight patients with AS and SP at the thoracolumbar area were treated sur...
متن کاملDiscovertebral destruction in ankylosing spondylitis complicated by spinal cord compression.
A 56 year old man with ankylosing spondylitis and discovertebral destruction presented with signs of spinal cord compression that was the result of the soft tissue reaction occurring at the level of the discovertebral destruction. This case emphasises the importance of early recognition, use of appropriate imaging techniques (computed tomographic myelography or magnetic resonance), and operativ...

A 73-year-old woman with suspected infectious diskitis was transferred to our institution. She had a 1 0-year history of ankylosing spondylitis and had had progressive kyphosis with intermittent pain in the thoracolumbar region for almost a year. She had no history of trauma, and her spine was nontender. She was afebrile, and her WBC count was normal. Plain films and tomograms of the thoracolum...

Background: Increased incidence of renal stone has been reported in ankylosing spondylitis (AS), but unlike some well-known renal involvements, they have not been fully studied. The aim of this study was to investigate the association of AS with urolithiasis and also the relation between urinary stone and severity markers. Methods: One hundred-sixty three AS patients were included in a cross-s...
