Clinical and Immunological Changes of Immunotherapy in Patients with Atopic Dermatitis: Randomized Controlled Trial

چکیده
Background. Immunotherapy has proven to be an useful tool in the management of allergic respiratory diseases; however, little has been studied in atopic dermatitis. Objective. To evaluate the clinical and immunological impact of immunotherapy with mites allergen extracts in atopic dermatitis. Methods. Patients with atopic dermatitis were assigned with computer-generated randomization to either of the following groups: (a) controls received only topical treatment with steroids and/or tacrolimus and (b) actively treated patients received topical treatment plus immunotherapy. Levels of serum total IgE, mites-specific IgE and IgG4 were assessed at study start and after one year of immunotherapy. Results. 31 patients in the active group and 29 in the control group completed the study. Symptoms and medication scores were significantly reduced in the active group after six months. Three patients in the control group showed new sensitizations to mites, while 3 patients in the active group showed neosensitization to shrimp with negative oral food challenge. We observed significant increase of mites-specific IgG4 levels in active group. Conclusion. Specific allergen immunotherapy induced a tolerogenic IgG4 response to mite allergens associated with favorable clinical effects in atopic dermatitis patients.

Cutaneous Leishmaniasis in Atopic Dermatitis Patients in Comparison with the Control Group in Kerman
Background: Atopic dermatitis is the most common chronic inflammatory dermatitis, due to immunological disorders. Patients with atopic dermatitis are prone to infectious diseases including bacterial infections. On the other hand, atopy may be protective against helminthes parasites. The objective of this study was to compare the frequency and clinical presentations of cutaneous leishmaniasis in...
متن کاملA short review on immunology of Atopic dermatitis
Atopic dermatitis (AD) is a pruritic, relapsing, chronic disease that arises most commonly during infancy, childhood, or adolescence. It is clear that genetic factors are important in AD, a family history of AD, allergic rhinitis, and/or asthma and other “Atopic” phenomena being common in patient with AD. The two main pathogenetic characteristic of AD are: I) IgE-mediated antigen dependent “Spe...
متن کاملSublingual immunotherapy in mite-sensitized children with atopic dermatitis: a randomized, double-blind, placebo-controlled study.
BACKGROUND Atopic dermatitis often has an allergic component, and immunotherapy may therefore prove beneficial. OBJECTIVE To assess the effect of sublingual immunotherapy (SLIT) in children with atopic dermatitis. METHODS Children age 5 to 16 years with atopic dermatitis (Scoring Atopic Dermatitis [SCORAD] > 7) and sensitization to dust mites alone, without food allergy or chronic asthma, w...

PURPOSE OF REVIEW To describe recent developments as well as the pros and cons of allergen specific immunotherapy as a putative therapeutic option for a subgroup of patients with atopic dermatitis. RECENT FINDINGS The question whether specific immunotherapy would impair or improve the severity and course of atopic dermatitis has been a matter of debate for a long time. Double-blind placebo-co...
متن کاملCommon Allergens in Patients with Atopic Dermatitis
Abstract Background and objective: Being exposed to different allergens, followed by the production of specific IgE, has an important role in causing atopic dermatitis, recognizing the allergens and applying immunotherapy for treatment. We aimed to determine the frequency of common allergens in the patients suffering from atopic dermatitis. Material and Methods: In this descriptive- anal...
