Surgery for mycotic aneurysm

چکیده
Backgound Infected aneurysm (or mycotic aneurysm) is defined as an infectious disease of the wall of an artery with formation of a blind, saccular out-pouching that is contiguous with the arterial lumen. Symptoms are frequently absent or non-specific during the early stages. Once clinically presented, infected aneurysms are often at an advanced stage of development and associated with complications such as rupture. Nontreatment or delayed treatment of infected aneurysms has a poor outcome, with high morbidity and mortality rate of fulminant sepsis or hemorrhage. In clinically suspected cases, computed tomography is state –ofthe-art technique used for diagnosis. Imaging features of infected aneurysms include a lobulated vascular mass, an indistinct irregular arterial wall, perianeurysmal edema, and a perianeurysmal soft-tissue mass. Urgent surgery performed for solving aortic rupture, carries high morbidity and mortality rates.

Mycotic aortic aneurysms account for 1–3% of all aortic aneurysms. The management of this disease is controversial. Since open surgical repair is associated with high morbidity and mortality rates, endovascular aneurysm repair is an alternative treatment method with promising early and midterm outcomes, although its long-term durability is unknown.Secondary aortoenteric fistulas may occur iatro...
متن کاملMycotic aortic aneurysm as a postsurgical complication: report of a case and review of the literature
Mycotic aneurysms are localized and irreversible dilatations of the arteries caused by weakening and damaging the arterial wall by an invasive organism establishing infective arteritis. Mycotic aneurysm of the thoracic aorta is a rare event; however, it can be fatal if not diagnosed early or not treated appropriately. Clinical findings are usually nonspecific; however, c...
متن کامل99mTc-RBC scan in evaluation of mycotic aneurysm [Persian]
Mycotic aneurysm is an uncommon but very important complication of subacute infectious endocarditis. It is usually asymptomatic but often causes sudden and massive bleeding with high mortality rate. 99mTc-RBC scan is a noninvasive method for detection of mycotic aneurysm throughout the body. In this report two cases with mycotic aneurysm are presented and the usefulness of 99mTc-RBC scan ...

Herein, we report a patient with arterio-venous fistula secondary to mycotic aneurysm, causing high output heart failure. The patient had one-year history of refractory heart failure and recurrent pulmonary edema. This presentation is a good example of curative surgically treatable cause of high-output congestive heart failure.Iran J Med Sci 2005; 30(1): 41-44. Keywords ● Arterio-venous fistula...
متن کاملRepair of mycotic coronary aneurysm without cardiopulmonary bypass: case report.
Mycotic aneurysms of the coronary arteries are extremely rare, with 15 cases reported. We report the successful treatment of a mycotic aneurysm of the left anterior descending coronary artery by coronary artery bypass grafting with aneurysm ligation and resection without the use of cardiopulmonary bypass.

Mycotic aneurysm of the thoracic aorta is a rare and life threatening condition. Two patients are presented (both male, aged 66 and 59 years) in whom coronary artery bypass surgery was complicated by the development of a mycotic aneurysm. Fever preceded the radiological and echocardiographic signs of the aneurysm by at least several months in both cases. Blood cultures were negative for one pat...
