چکیده

PRIMARY OBJECTIVE To present a novel pathophysiological approach to acute concussion and post-concussion syndrome (PCS). RESEARCH DESIGN Review of the literature. METHODS AND PROCEDURES PubMed searches were performed to identify articles related to the pathophysiology and treatment of concussion and PCS. Relevant articles that contributed to the primary objective of the paper were included. MAIN OUTCOME AND RESULTS This paper presents an evidence-based approach to acute concussion and PCS that focuses on the identification of specific post-concussion disorders (PCDs) caused by impairments in global brain metabolism (Physiologic PCD) or neurological sub-system dysfunction (Vestibulo-ocular PCD and Cervicogenic PCD) that can be distinguished by features of the clinical history, physical examination and treadmill exercise testing. This novel approach also allows for the initiation of evidence-based, multi-disciplinary therapeutic interventions that can improve individual symptoms and promote efficient neurological recovery. CONCLUSION Future studies incorporating neuro-imaging and exercise science techniques are underway at the author's institutions to validate this novel pathophysiological approach to acute concussion and PCS.

The neuro-ophthalmology of head trauma.

SUMMARY Traumatic brain injury (TBI) is a major cause of morbidity and mortality. Concussion, a form of mild TBI, might be associated with long-term neurological symptoms. The effects of TBI and concussion are not restricted to cognition and balance. TBI can also affect multiple aspects of vision; mild TBI frequently leads to disruptions in visual functioning, while moderate or severe TBI often...

Manual therapy for cervicogenic dizziness: Long-term outcomes of a randomised trial.

Manual therapy is effective for reducing cervicogenic dizziness, a disabling and persistent problem, in the short term. This study investigated the effects of sustained natural apophyseal glides (SNAGs) and passive joint mobilisations (PJMs) on cervicogenic dizziness compared to a placebo at 12 months post-treatment. Eighty-six participants (mean age 62 years, standard deviation (SD) 12.7) with...

Brain or strain? Symptoms alone do not distinguish physiologic concussion from cervical/vestibular injury.

OBJECTIVE To compare symptoms in patients with physiologic postconcussion disorder (PCD) versus cervicogenic/vestibular PCD. We hypothesized that most symptoms would not be equivalent. In particular, we hypothesized that cognitive symptoms would be more often associated with physiologic PCD. DESIGN Retrospective review of symptom reports from patients who completed a 22-item symptom questionn...

Postnatal development of spatial coding in the gravity sensing system

The critical maturation time of central otolith neurons in processing spatial orientations was examined in Sprague-Dawley rats. With the use of immuno-hybridization histochemical methods, we observed c-fos expression in vestibular nuclear neurons responding to transverse movement on the horizontal plane as early as P7 and those to antero-posterior stimulation as early as P9. In the inferior oli...

Effect of Manual Therapies on Parameters of Post- Concussion Symptoms Recovery : A Systematic Review

Objectives: Post- concussion syndrome (PCS) is a syndrome resulting from an injury to the head leading to loss of consciousness. It includes several symptoms like fatigue, headache, dizziness and difficulty in concentration. Athletes possess a major threat to PCS and its treatment is still considered controversial. The objective of this systematic review is to study the impact of manual therapi...
