چکیده

T he Wright peak expiratory flow meter was introduced in 1959 and provided a portable piece of equipment for assessing some aspects of lung function in the ambulatory care setting. The original device has been supplanted by the mini Wright peak flow meter (and a range of other devices) which have become relatively cheap but are reasonably reliable for home monitoring. So, with an available device for measuring peak expiratory flow (PEF), what is its role in the 21st century? PEF reflects a range of physiological characteristics of the lungs, airways, and neuromuscular characteristics of individuals. These include lung elastic recoil, large airway calibre, lung volume, effort, and neuromuscular integrity. The reflection of airway calibre makes the PEF meter suitable for measuring variation in PEF over time to provide support for:

Effects of Ramadan fasting on spirometric values and clinical symptoms in asthmatic patients

Introduction: Ramadan is the 9th  Islamic lunar month during which Muslims avoid eating and drinking from sunrise to sunset.  The effect of Ramadan intermittent fasting on asthma control is controversial.  The aim of this study was to investigate the effects of Ramadan fasting on the spirometric variables and clinical symptoms on well-controlled asthmatic patients during Ramadan. Material and M...

Peak expiratory flow: conclusions and recommendations of a Working Party of the European Respiratory Society.

The use of peak flow meters has been widely adopted for monitoring patients with asthma. The Working Party of the European Respiratory Society (ERS) has solely addressed technical and physiological issues relating to peak expiratory flow (PEF) (flow describes the rate of change of volume (volume rate), so that flow rate is equivalent to volume acceleration. Hence, PEF should be used in preferen...

Changes in peak expiratory flow indices as a proxy for changes in bronchial hyperresponsiveness. Dutch Chronic Non-Specific Lung Disease study group.

Guidelines for asthma management advocate home peak expiratory flow (PEF) monitoring. It is commonly stated that PEF variability is a good proxy of bronchial hyperresponsiveness (BHR), a hallmark of asthma. However, this has hardly been tested longitudinally, as required to monitor asthma. This study assesses which PEF index correlates best with BHR longitudinally and whether the correlation im...

Compliance with and accuracy of daily self-assessment of peak expiratory flows (PEF) in asthmatic subjects over a three month period.

Serial peak expiratory flow (PEF) assessment has been proposed in the clinical evaluation of asthma. In subjects attending the asthma clinic of a tertiary care hospital, we wanted to assess: 1) compliance in performing PEF; and 2) accuracy of a PEF-diary. Twenty adult asthmatic subjects, all using inhaled steroids, were asked to assess their PEF in the morning and evening with a VMX instrument ...

Standardization of ambulatory peak flow monitoring: the importance of recent beta2-agonist inhalation.

Standardization of conditions for peak expiratory flow (PEF) monitoring is much more difficult in practice than for laboratory spirometry. Patients are usually asked to record PEF before medication. The aim of this study was to determine the effect of prior bronchodilator use on PEF outcome measures in a clinical trial. Electronic PEF records from 43 subjects with poorly controlled asthma were ...
