چکیده

to the editor: Zile et al. (May 6 issue) 1 conclude that heart failure develops in patients with a normal ejection fraction because of abnormal active and passive diastolic function. However, the applicability of this conclusion to all such patients is uncertain for several reasons. First, accurate assessment of left ventricular volume, on which the conclusions critically depend, requires a three-dimensional imaging technique that measures the long axis 2,3 ; two-dimensional techniques are inadequate for this purpose. Second, the majority of the patients studied were men, and on average, they were less than 60 years of age. However, most patients with a normal ejection fraction in whom heart failure develops are elderly women. 4 Third, although the underlying disease in patients with heart failure and a normal ejection fraction differs according to whether hypertension is or is not present, data from these two groups were pooled, precluding the identification of different pathophysiological mechanisms. Fourth, the results do not account for data already in the literature showing that there are rightward, downward shifts of the diastolic pressure–volume relation in some patients with heart failure and a normal ejection fraction. 5 Therefore, the conclusions reached by Zile et al. may not apply to most patients with heart failure and a normal ejection fraction. The mechanisms responsible for this disorder in most patients still need to be elucidated.

Changes in the structure of the heart and blood pressure after a period of concurrent endurance-resistance training in patients with chronic heart failure

Background: Exercise improves functional capacity by improving endothelial function and musculoskeletal aerobic metabolism, but its effects on hemodynamic control are not well known. The aim of this study was to investigate the effect of aerobic and resistance trainings on structural changes and blood pressure in patients with heart failure. Materials and methods: 76 patients aged 65 to 79 year...

بررسی فراوانی نشانگر CTnI و عوامل مرتبط با آن در بیماران با دیاستولیک دیس‌فانکشن AHF (PEF)

Abstract Background: Diastolic heart failure comprises 50 percent of cases of heart failure. The implications of increased levels of cardiac troponin I (CTnI) on congestive heart failure with preserved systolic function (AHF PEF) have been poorly evaluated. We hypothesized that its level might be elevated in AHF PEF so this evaluation was done in patients referred to Taleghani hospital with ...

Prevalence of Heart Failure in the Cases of Beta-thalassemia Major; Two Years Follow-Up

Inroduction: Heart failure (HF) is an important cause of morbidity and mortality in the cases of Beta-thalassemia major. The purpose of this study was to estimate HF prevalence in these patients and to assess the survivability of those who were treated with intensive chelating therapy.   Design and methods: This cross sectional study included 72 beta-thalassemia major cases, the mean age at t...

New concepts in diastolic dysfunction and diastolic heart failure: Part I: diagnosis, prognosis, and measurements of diastolic function.

There is growing recognition that congestive heart failure (CHF) caused by a predominant abnormality in diastolic function (ie, diastolic heart failure) is both common and causes significant morbidity and mortality. However, there is continued controversy surrounding the definition of diastolic dysfunction and the diagnostic criteria for diastolic heart failure. As a result, clinical therapeuti...

Diagnosis and management of diastolic dysfunction and heart failure.

Diastolic heart failure occurs when signs and symptoms of heart failure are present but left ventricular systolic function is preserved (i.e., ejection fraction greater than 45 percent). The incidence of diastolic heart failure increases with age; therefore, 50 percent of older patients with heart failure may have isolated diastolic dysfunction. With early diagnosis and proper management the pr...

Left and right ventricular diastolic dysfunction and diastolic heart failure: does one lead to the other?

Background and Objective Diastolic dysfunction of the left ventricle is a mechanical abnormality diagnosed primarily by echocardiogram, and can be distinguished into three separate degrees based on the severity of reduction in passive compliance and active myocardial relaxation. Methods A literature search was performed for basic science studies, clinical studies and major practice guidelines o...
