چکیده

Copyright © 2009 by the Congress of Neurological Surgeons Hydrocephalus is a neurological disorder that can result in significant disability or even death, regardless of age, sex, or any other individual characteristic. Because of the complex interaction between intracranial pressure (ICP) and hydrocephalus, no clear conclusions can be drawn from the magnitude of ICP alone. Uncontrolled and increased ICP is one of the most common causes of neurological death, whereas patients with large spaceoccupying lesions may have clinical signs of neurological distortion (e.g., brainstem) without significant elevation of ICP (17, 23). The phenomena of local or global effects such as ventricular dilation, raised ICP, and a depleted pressurevolume relationship are textbook concepts (5, 33). These may be regarded as symptoms that reflect the presence of a spaceoccupying process, which can no longer be compensated for. Consequently, subsidiary INDEX OF CEREBROSPINAL COMPENSATORY RESERVE IN HYDROCEPHALUS

Cerebrospinal compensation of pulsating cerebral blood volume in hydrocephalus.

OBJECTIVE The aim of the study was to develop a computational method for the assessment of brain pressure compensation of cerebrospinal arterial blood inflow. The method was verified using clinical recordings performed during infusion studies in a group of patients diagnosed with hydrocephalus. MATERIALS AND METHODS We studied 27 patients suspected of having normal pressure hydrocephalus. The...

Testing of cerebrospinal compensatory reserve in shunted and non-shunted patients: a guide to interpretation based on an observational study.

OBJECTIVE To design a computerised infusion test to compensate for the disadvantages of Katzman's lumbar infusion method: inadequate accuracy of estimation of the resistance to cerebrospinal fluid outflow and poor predictive value in normal pressure hydrocephalus. METHODS Accuracy was improved by intracranial pressure signal processing and model analysis for measurement of cerebrospinal compe...

Assessment of Cerebral Hemodynamics in Traumatic Brain Injury

We employ complex continuous wavelet transforms to develop a consistent mathematical framework capable of quantifying both cerebrospinal compensatory reserve and cerebrovascular pressure–reactivity. The wavelet gain ( ) ICP ABP f η , defined as the frequency dependent ratio of time averaged wavelet coefficients of intracranial pressure (ICP) and arterial blood pressure (ABP) fluctuations, chara...

How does CSF dynamics change after shunting?

OBJECTIVE Hydrocephalus is much more complex than a simple disorder of cerebrospinal fluid (CSF) circulation. Shunting primarily corrects disturbed fluid flow which may have an impact on cerebral blood flow and metabolism. We studied hydrocephalic patients before and after shunting to characterize changes in their CSF compensatory parameters. MATERIAL AND METHODS We selected 25 patients and s...

State-of-the-art monitoring in treatment of dengue shock syndrome: a case series

BACKGROUND Early recognition and treatment of circulatory volume loss is essential in the clinical management of dengue viral infection. We hypothesized that a novel computational algorithm, originally developed for noninvasive monitoring of blood loss in combat casualties, could: (1) indicate the central volume status of children with dengue during the early stages of "shock"; and (2) track fl...
