چکیده

In addition to self-limited myotoxicity, statins have recently been shown to trigger an immune-mediated necrotizing myopathy (IMNM), which is distinguished from polymyositis (PM) and dermatomyositis (DM) by the absence of primary inflammation on muscle biopsy. Previously, we have shown that patients with statin-associated necrotizing myopathy express an autoantibody targeting 3-hydroxy-3methylglutaryl-CoA reductase (HMGCR), the pharmacological target of statins (1,2). We conducted a case-control study to characterize the comorbidities and detailed individual statin history and to investigate the possible clinical associations that could contribute to the development of anti-HMGCR– positive myopathy in statin-treated patients.

Autoantibodies against 3-hydroxy-3-methylglutaryl-coenzyme A reductase in patients with statin-associated autoimmune myopathy.

OBJECTIVE In addition to inducing a self-limited myopathy, statin use is associated with an immune-mediated necrotizing myopathy (IMNM), with autoantibodies that recognize ∼200-kd and ∼100-kd autoantigens. The purpose of this study was to identify these molecules to help clarify the disease mechanism and facilitate diagnosis. METHODS The effect of statin treatment on autoantigen expression wa...

Analysis of Autoantibodies to 3-Hydroxy-3-methylglutaryl-coenzyme A Reductase Using Different Technologies

Diagnostic tests are needed to aid in the diagnosis of necrotizing myopathies associated with statin use. This study aimed to compare different technologies for the detection of anti-HMGCR antibodies and analyze the clinical phenotype and autoantibody profile of the patients. Twenty samples from myositis patients positive for anti-HMGCR antibodies using a research addressable laser bead assay a...

Anti-HMGCR antibodies in systemic sclerosis

The objective of this study was to investigate the frequency of autoantibodies to hydroxymethylglutaryl coenzyme A reductase (HMGCR) in systemic sclerosis (SSc) and associations with inflammatory myositis and statin exposure. This was a cross-sectional, multicenter study of 306 subjects from the Canadian Scleroderma Research Group cohort who had complete data on statin exposure and serology for ...

Anti-HMGCR Myopathy

Anti-HMGCR myopathy was first recognized and characterized in patients with a history of statin exposure and immune-mediated necrotizing myopathy. After the discovery of anti-HMGCR autoantibodies, several international groups identified and characterized more patients, expanding the phenotypic spectrum of this disease to include pediatric patients and young adults without statin exposure and th...

Cancer association as a risk factor for anti-HMGCR antibody-positive myopathy

OBJECTIVE To show cancer association is a risk factor other than statin exposure for anti-3-hydroxy-3-methylglutaryl coenzyme A reductase autoantibody-positive (anti-HMGCR Ab+) myopathy. METHODS We analyzed the clinical features and courses of 33 patients (23 female and 10 male) with anti-HMGCR Ab+ myopathy among 621 consecutive patients with idiopathic inflammatory myopathies. RESULTS Amon...
