The comparative antiarrhythmic actions of lidocaine and its quarternary derivative, methyl lidocaine.

چکیده
Lidocaine has proved to be an effective antiarrhythmic agent in the management of ventricular arrhythmias subsequent to acute myocardial infarction. Lidocaine's short duration of action and its propensity for producing central nervous system (CNS) stimulation suggest that a safer and more effective therapeutic agent, based on a modification of the lidocaine structure, might be found. The quaternary ammonium compound, methyl lidocaine, was synthesized and its actions in experimentally-induced arrhythrnias were studied and compared to those of lidocaine. The antiarrhythmic effects of lidocaine and methyl lidocaine were examined in the anesthetized dog against ouabain-induced ventricular tachycardia and in conscious animals with ventricular tachycardia 48 hours after two-stage ligation of the anterior descending coronary artery. Both pharmacological agents were capable of reversing digitalis-induced arrhythmias and restoring normal sinus rhythm in animals 48 hours after surgical ligation of the anterior descending coronary artery, but methyl lidocaine remained effective for a significantly longer period and its continual administration was not associated with signs of CNS toxicity. In experiments designed to determine the electrical fibrillation threshold, both drugs were able to reduce the vulnerability to ventricular fibrillation but the time course of action for each drug differed. Lidocaine had an immediate effect of increasing the ventricular fibrillation threshold whereas the peak effect of methyl lidocaine was delayed and the increase in the fibrillation threshold lasted longer. This study concludes that the quaternary derivative of lidocaine, methyl lidocaine, possesses antiarrhythmic properties similar to those of lidocaine, but differs in its duration of action and lack of overt stimulatory effects upon the central nervous system.

Effect of Diphenylhydantoin and Lidoca1ne on Cardiac Arrhythmias Induced by Hypothalamic Stimulation
The importance of the central nervous system in the antiarrhythmic actions of diphenyhiydantoin and lidocaine was studied using arrhythmias induced by electrical stimulation of the posterior hypothalamus in cats. Hypothalamic stimulation resulted in cardiac arrhythmias mediated by the sympathetic nervous system. The arrhythmias occurred during stimulation, and recordings from cardiac sympatheti...
متن کاملKinetic Effects of Quarternary Lidocaine Block of Cardiac Sodium Channels" A Gating Current Study
The interaction of antiarrhythmic drugs with ion channels is often described within the context of the modulated receptor hypothesis, which explains the action of drugs by proposing that the binding site has a variable affinity for drugs, depending upon whether the channel is closed, open, or inactivated. Lack of direct evidence for altered gating of cardiac Na channels allowed for the suggesti...

متن کاملInhibition of cardiac sympathetic nerve activity during intravenous administration of lidocaine.
The antiarrhythmic action of lidocaine has been attributed solely to its direct electrophysiological effects on the heart. However, lidocaine is particularly effective in treating ventricular arrhythmias associated with increased sympathetic activity, e.g., in myocardial infarction and digitalis toxicity. We tested the hypothesis that lidocaine administered intravenously depressed cardiac sympa...
متن کاملA qualitative distinction between the beta-receptor-blocking and local anesthetic actions of antiarrhythmic agents.
Fragmented cardiac sarcoplasmic reticulum was isolated by differential centrifugation from canine myocardium. The effects of propranolol, quinidine, lidocaine (lignocaine) and tetracaine (amethocaine) on calcium uptake in sarcoplasmic reticulum in vitro were then measured. All the drugs are antiarrhythmic agents with local anesthetic activity, but propranolol and quinidine are known also to hav...
