چکیده

See-saw nystagmus is a rare ocular abnormality, only 15 cases having been recorded to date (Maddox, 1914; Rucker, 1946; Larsen, 1957; Jensen, 1959; Smith and Mark, 1959; Mark, Smith, and Kjellberg, 1960; Lourie, 1963; Schurr, 1963; Kinder and Howard, 1963; Slatt and Nykiel, 1964; and Daroff, 1965). As initially described by Maddox (1914), the eye movements consist of regular reciprocating oscillations in which one eye rises while the other falls. Torsion of the eyes accompanies the vertical movements so that the axis of the rising eye intorts while that of the falling eye extorts. Nine of the reported cases of see-saw nystagmus have been associated with tumours in the region of the optic chiasm and diencephalon. These lesions do not entirely explain the pathogenesis of the seesaw movement, however, since in other cases brainstem or ocular lesions were described instead. Furthermore, in innumerable parasellar lesions seesaw nystagmus does not occur. Much still remains to explain the pathophysiology of see-saw ocular movements. The purpose of the present communication is to present an additional case in which see-saw nystagmus was observed in a patient with bitemporal hemianopia and abnormal vestibular responses. Attention is directed to several of the previously reported cases in which abnormal vestibular responses were also noted. A mechanism is suggested by which see-saw nystagmus is produced by various combinations of visual, vestibular, or other lesions rather than by a lesion in a single anatomical site.

Two additional scenarios for see-saw nystagmus: achiasma and hemichiasma.

The discovery of canine achiasma and hemichiasma has been followed by the identification of human achiasma (four individuals, to date). See-saw nystagmus was present in all cases of achiasma (canine and human) and in one of two cases of canine hemichiasma studied. Human infants with see-saw nystagmus should be imaged for possible structural abnormalities of the optic chiasm.

Mystery Case: pendular see-saw nystagmus as a delayed complication of traumatic brain injury.

NYSTAGMUS AS A DELAYED COMPLICATION OF TRAUMATIC BRAIN INJURY Eric R. Eggenberger, East Lansing, MI: I read with interest the delayed see-saw nystagmus case detailed by Yunusov et al. We also published 2 delayed cases occurring 21 and 37 years post head trauma involving the chiasmal region. Both patients exhibited bitemporal hemianopia and MRIs suggestive of chiasmal disruption; one case experi...

Teaching video neuroimages: see-saw nystagmus.

A 43-year-old woman experienced a worsening of her usual headache. As a right hemifacial hypoesthesia had appeared, the patient underwent a brain MRI that revealed a giant cavernoma localized at the left meso-diencephalic region (figure, A and B). After the operation, an involuntary ocular oscillation known as see-saw nystagmus developed (video on the Neurology® Web site at www.neurology.org). ...

BMJ Case Rep Published online: [please include Day Month Year] doi:10.1136/ bcr-2014-206851 DESCRIPTION A 49-year-old healthy woman presented with sudden onset of binocular diplopia and unsteady gait. Physical examination showed convergence-retraction nystagmus on up-gaze and see-saw nystagmus on left-gaze (video 1). She also had ocular tilt reaction (OTR) with skew devi...

The congenital and see-saw nystagmus in the prototypical achiasma of canines: comparison to the human achiasmatic prototype

We applied new methods for canine eye-movement recording to the study of achiasmatic mutant Belgian Sheepdogs, documenting their nystagmus waveforms and comparing them to humans with either congenital nystagmus (CN) alone or in conjunction with achiasma. A sling apparatus with head restraints and infrared reflection with either earth- or head-mounted sensors were used. Data were digitized for l...
