چکیده

T herapeutic decisions in patients with coronary artery disease are frequently based on the severity of chest pain despite the lack of correlation between severity of symptoms and prognosis. There are many circumstances in which objective evidence for myocardial ischemia is present but symptoms are absent. Episodes of so-called “silent myocardial ischemia” are very frequent in some patients with other manifestations of coronary heart disease; for example, patients with angina have no symptoms in 70 to 80 percent of all recorded ischemic episodes. While the overall incidence, spontaneous variability, and relationship of symptomatic to asymptomatic ischemia are largely unknown, this condition is being increasingly recognized. The purpose of this review is to discuss the clinical aspects of silent ischemia and the rationale for treatment (Fig 1). This information can be used to develop a strategy to help the clinician deal with this problem.

Silent myocardial ischemia.

Silent myocardial ischemia is increasingly recognized as a common phenomenon in a variety of people with coronary artery disease. The natural history is poorly understood, but available data suggest that silent ischemia adversely affects prognosis in all groups studied. Based on this information, we believe that efforts should be directed at detecting and aggressively treating silent ischemic e...

Application of Anatomical and Functional Modalities in Detection of Silent Myocardial Ischemia in Asymptomatic Diabetic Patients- A Review Article

Cardiovascular disease (CAD) screening in asymptomatic patients with diabetes mellitus is controversial. An exercise stress test is a safe and effective initial testing for Ischemic Heart Disease (IHD) screening. Treadmill stress test is not the best screening method for a particular patient who is not able to exercise, abnormal electrocardiogram (ECG) or positive exercise treadmill test (ETT) ...

Diagnostic Value of Ankle Brachial Index for Myocardial Ischemia in Asymptomatic Diabetic Patients and Comparison with Myocardial Perfusion Imaging

Objective: The ankle-brachial index (ABI) as a simple test which can detect peripheral arterial disease (PAD). Therefore in this study we try to evaluate the diagnostic value of ABI for silent myocardial ischemia in diabetic patients and compare the results with myocardial perfusion imaging (MPI) results. Materials and Methods: All 149 diabetic patients in this study were categorized according...

[Predictive value of differential pulse pressure in the diagnosis of silent myocardial ischemia in patients with type-2 diabetes].

Silent myocardial ischemia occurs more frequently in diabetics. Differential arterial pulse pressure is a valuable predictor of cardiovascular disease. We studied 48 consecutive male patients with type-2 diabetes and no known history of ischemic heart disease. Ambulatory monitoring of arterial pressure was carried out and the presence of silent myocardial ischemia was studied using a protocol t...
