Clinical trial design and end points for epidermal growth factor receptor-targeted therapies: implications for drug development and practice.

چکیده
Introduction There is an increased perception that current chemotherapeutic drugs have almost reached a ceiling as systemic anticancer therapies. In great part because of their unpredictable side effects, the development of these cytotoxic drugs followed certain patterns and principles that may not necessarily apply to modern molecule-targeted antineoplastic strategies. In the following commentary, some of these differences will be highlighted. Using the EGFR network as a model of a therapeutic target in cancer, key aspects that apply to the clinical development of EGFR-directed drugs will be discussed. These issues may well apply to many other molecule-targeted strategies in preclinical and clinical development.

Molecular Docking Based on Virtual Screening, Molecular Dynamics and Atoms in Molecules Studies to Identify the Potential Human Epidermal Receptor 2 Intracellular Domain Inhibitors
Human epidermal growth factor receptor 2 (HER2) is a member of the epidermal growth factor receptor family having tyrosine kinase activity. Overexpression of HER2 usually causes malignant transformation of cells and is responsible for the breast cancer. In this work, the virtual screening, molecular docking, quantum mechanics and molecular dynamics methods were employed to study protein–ligand ...
متن کاملTargeted Therapies in Breast Cancer: Implications for Advanced Oncology Practice
The systemic therapeutic management of breast cancer has undergone significant transformation in the past decade. Without targeted therapies, conventional treatment with cytotoxic agents has reached the limit of its potential in terms of patient survival for most types of cancer. Enhanced understanding of the pathogenesis of tumor cell growth and metastasis has led to the identification of sign...
متن کاملTargeted therapies for the treatment of breast cancer in the post-trastuzumab era.
Targeted therapies for breast cancer are evolving rapidly. Trastuzumab has revolutionized breast cancer treatment and outcome, reducing the risk for recurrence and significantly increasing survival, at least for a subgroup of patients. Other targeted therapies, such as bevacizumab, a monoclonal antibody targeting angiogenesis, lapatinib, a dual human epidermal growth factor receptor (HER)-1 and...
متن کاملChallenges and future of biomarker tests in the era of precision oncology: Can we rely on immunohistochemistry (IHC) or fluorescence in situ hybridization (FISH) to select the optimal patients for matched therapy?
Molecular techniques have improved our understanding of the pathogenesis of cancer development. These techniques have also fueled the rational development of targeted drugs for patient populations stratified by their genetic characteristics. These novel methods have changed the classic paradigm of diagnostic pathology; among them are IHC, FISH, polymerase chain reaction (PCR) and microarray tec...
متن کاملProgress of clinical research on targeted therapy combined with thoracic radiotherapy for non-small-cell lung cancer
The combination of radiotherapy and targeted therapy is an important approach in the application of targeted therapy in clinical practice, and represents an important opportunity for the development of radiotherapy itself. Numerous agents, including epidermal growth factor receptor, monoclonal antibodies, tyrosine kinase inhibitors, and antiangiogenic therapies, have been used for targeted ther...
