Type-III hereditary angioedema resolved by surgery

چکیده
Background In classic hereditary angioedema, inadequate C1-inhibitor (C1-INH) failing to restrict factor-XII activity leads to increased production of bradykinin, a potent vasodilator and mediator of angioedema. Hereditary angioedema with normal C1-INH (Type-III) manifests with sporadic recurrent angioedema but normal C1-INH concentration and activity. Here, bradykinin accumulation appears dependent on Factor XII and Factor XII gene mutations are sometimes found. Type-III angioedema affects almost exclusively females, worse in pregnancy, on oral contraceptives as estrogen may increase total bradykinin. Diagnosis is difficult given the clinical heterogeneity and lack of biochemical indicators. Treatment carries various risks when given for prophylaxis and a challenge for timely administration in an acute crisis. This is the first case report in the literature of a woman whose repeated angioedema episodes resolved with surgical resection of an ovarian cyst.

Hereditary angioedema is a rare disorder of complement system which is often seen with autosomal dominant hereditary. Clinical characteristics include non- pruritic and non-pitting mucocutaneous edema that could involve all parts of the body. This study reports seven cases of hereditary angioedema with classical manifestations accompanied by low function of C1INH (type 2). One death occurred du...
متن کاملAngioedema due to Systemic Isotretinoin Therapy
Angioedema is the swelling of the mucosal membranes as a variant of urticaria induced by hereditary C1 esterase inhibitor enzyme deficiency, certain foods, or drugs. Herein, we report the case of a 23-year-old woman, with mild-moderate acne presenting with widespread facial angioedema on the 2nd day of systemic isotretinoin treatment. The patient had taken no drugs other than isotretinoin in th...
متن کاملHereditary angioedema. Long-term follow-up of 88 patients. Experience of the Argentine Allergy and Immunology Institute.
Since the detection of the first patient with hereditary angioedema (HA) in 1978, 88 new patients belonging to 16 families have been referred to our clinic. Eighty patients had Type I disease, 5 Type II, and 3 Type III (secondary). We describe the clinical onset, frequent complications, diagnostic tests of the complement system, and abnormalities of the coagulation pathway linked to complement ...

Background Hereditary angioedema (HAE) is a disease transmitted by autosomal dominant inheritance, characterized by quantitative and / or functional deficiency of C1 inhibitor (C1-INH), which causes episodes of swelling, with involvement of many organs. HAE is currently divided into three groups. The HAE type III is a less frequent disorder that mainly affects women and is characterized by norm...
