Bilateral thalamic infarction following transsphenoidal surgery.

چکیده
The transsphenoidal approach is widely employed for access to surgical lesions of the sella. Following the introduction of the operating microscope to the procedure by Hardy,1 it has become a safe and well-established operation, as demonstrated by the low morbidity and mortality in several large case series over the past four decades.2-6 Nevertheless, rare but serious complications, most notably injury to the intracavernous carotid artery7,8 and post-operative meningitis,9 do occur and have been well-documented. In this article we present, to our knowledge, the first reported case of bilateral thalamic infarction occurring as a serious complication of transsphenoidal surgery for resection of a pituitary adenoma. We also briefly review the pathogenesis of bilateral thalamic infarctions, and discuss the intraoperative factors unique to this case and to the transsphenoidal approach which may have contributed to this unusual complication.

Hypersexuality following bilateral thalamic infarction: case report.
Hypersexuality is a rare but well recognized condition following brain injury. It has been described secondarily to dysfunction in the hypothalamus, the temporal and frontal lobes. We report a 63 year-old man that developed neuropsychological disturbances with hypersexuality as a prominent feature, disinhibition and moderate memory loss, hypersomnia and irritability after a bilateral paramedian...
متن کاملMoyamoya Syndrome Associated with Basal Meningioma Successfully Treated by the Modified Transsphenoidal Approach: Case Report
Objective Moyamoya vasculopathy associated with skull base tumors has been rarely reported except for partially removed pituitary adenoma and craniopharyngioma. Only a single case of meningioma resulted in vast cerebral infarction. Surgical treatment carries a high risk of damage to the compensatory collateral circulation through leptomeningeal anastomosis leading to a devastating outcome. Case...
متن کاملEarly uncoupling of cerebral blood flow and metabolism after bilateral thalamic infarction.
Cerebral blood flow and metabolism may be uncoupled in the early phases after stroke onset. Prior reports of bilateral thalamic stroke have described subsequent coupling of blood flow and metabolism during the chronic stage. We chronicled the evolving relationship of blood flow and metabolism with concomitant single photon emission CT and positron emission tomography from the subacute to chroni...
متن کاملProduction of various models of cerebral infarction in the dog by means of occlusion of intracranial trunk arteries.
Using the dog, which has been believed unsuitable for research on brain infarction because of an extensive collateral cerebral circulation, we have succeeded in producing at will ischemic foci, as determined from post-occlusion carbon perfusion, in the thalamus, cerebral mantle or entire cerebral hemisphere. This has been achieved by occlusion of various combinations of cerebral vessels at the ...
متن کاملThalamic infarction following a Russell's viper bite.
We report a case of a Russell's viper (Daboia russelii) bite involving a 55-year-old male who developed a bilateral thalamic infarction. Although the coagulopathy was controlled within twenty-four hours, the patient became restless and disoriented. Due to the initial prolonged clotting time, we suspected an intracranial bleed. T2 magnetic resonance imaging (MRI) of the brain showed bilateral in...
