The Emphysema Response to Forced Straining
نویسنده
چکیده
R ECENTLY there has been a renewed interest in the physiologic effects of forced straining (Valsalva's maneuver). Particular emphasis has been placed on the blood pressure changes during and immediately after forced straining in an attempt to define more accurately the diagnosis of certain circulatory abnormalities, particularly hemodynamically significant mitral stenosis and the presence of congestive failure.1-7 Little attention has been given to the manner in which pulmonary disease modifies the response to straining. The purpose of this paper is to present data obtained from observing circulatory changes during and immediately after Valsalva's maneuver in patients with pulmonary emphysema and to contrast these changes with responses in patients with normal and diseased hearts. The "emphysema response" is unique and to our knowledge has not been described except in our recent preliminary report.8

متن کاملResponse to corticosteroids in chronic airflow obstruction: relationship to emphysema and airways collapse.
We have studied the relationship between emphysema and airways collapse, and response to corticosteroids in patients with chronic airflow obstruction. One hundred and seven patients completed a placebo-controlled trial comparing 2 wks treatment with oral prednisolone 40 mg.day-1 to inhaled beclomethasone dipropionate 500 micrograms t.d.s. Response to corticosteroids was defined on the basis of ...
متن کاملThe use of expiratory forced flows for determining response to bronchodilator therapy.
Two hundred seventeen patients were studied before and after administration of isoproterenol by IPPB. In 35 patients, lung specimens were examined within 12 months after the study. Patients with elevated bronchial gland-bronchial wall ratio (Reid index showed significantly more improvement in forced expiratory flows after bronchodilator therapy, as compared to patients with normal Reid index. T...
متن کاملPneumomediastinum Due to Intractable Hiccup as the Presenting Symptom of Multiple Sclerosis
Pneumomediastinum and subcutaneous emphysema generally occurs following trauma to the esophagus or lung. It also occurs spontaneously in such situations of elevating intra-thoracic pressure as asthma, excessive coughing or forceful straining. We report here on the rare case of a man who experienced the signs of pneumomediastinum and subcutaneous emphysema after a prolonged bout of intractable h...
متن کاملCervicofacial Surgical Emphysema following Tonsillectomy
We report the case of a patient who developed cervicofacial subcutaneous emphysema following a routine tonsillectomy. An 18-year-old male with swallowing difficulties underwent a tonsillectomy and developed swelling of the right side of his neck and face 36 hours after surgery. A neck X-ray revealed subcutaneous emphysema. Unlike similar previously published cases, there were no postoperative i...
