Non-atheromatous causes of cerebral infarction.

چکیده

The 600-fold increase in the incidence of stroke with advancing age, from 10 per 100,000 below 35 years of age to 6,000 per 100,000 over 75 years (Kurtzke, 1976), is related in the main to the creeping peril of atherosclerosis. However, only a minority of strokes before middle-age can be ascribed to atheroma and in every age-group non-atheromatous causes of cerebral infarction account for a small but significant proportion of vascular lesions. Two fascinating reports, presented in this issue of the Postgraduate Medical Journal by Salanova and Taubner and by Monton et al., provide a reminder that extraordinary and preventable aetiological factors may precipitate sudden hemiplegia. The diversity of aetiologies of cerebral infarction reflects the changing patterns of modern civilization: our life-style includes many hazardous occupations and leisure pursuits, the excesses of society range from drug abuse to fitness fanaticism, and medical survival is achieved at the cost of occasional but unanticipated side effects ofyet more potent therapeutic agents (Chambers et al., 1981). One result has been an increase in the incidence of non-fatal strokes in the decade 1970-9 (Lindstrom, Soderstrom and Mettinger, 1982).

Predisposing factors for cerebral infarction: the Oxfordshire community stroke project.

The frequency of known causative factors of cerebral infarction was studied in 244 cases of first ever stroke due to cerebral infarction proved by computed tomography or at necropsy who were registered in the first two years of a prospective community based study. Risk factors for cerebral infarction were present in 196 (80%) cases; hypertension in 126 (52%); ischaemic heart disease in 92 (38%)...

Branch Atheromatous Plaque: A Major Cause of Lacunar Infarction (High-Resolution MRI Study)

BACKGROUND Lacunar infarctions account for up to 25% of all ischemic strokes and, thus, constitute a numerically important subgroup. It is important that the two pathogeneses of lacunar infarction, that is, small-vessel occlusion and branch atheromatous disease, be differentiated because prognoses and treatment strategies differ. The authors evaluated the presence of branch atheromatous plaque ...

Variable angiographic findings in patients with stroke and neurosyphilis.

We describe four patients with cerebral infarction and active neurosyphilis who had variable angiographic findings. Patients 1 and 2 had evidence of arteritis and an atheromatous ulcerated stenosis of the extracranial carotid artery, respectively. The third and fourth patients had symptoms of lacunar infarction, but cerebral angiography was normal in patient 3, whereas patient 4 had evidence of...

The carotid bifurcation plaque: pathologic findings associated with cerebral ischemia.

Embolization from or decreased flow through cervical carotid and vertebral arteries causes ischemic stroke syndromes. Specific pathologic findings were studied in 50 symptomatic patients who underwent 69 carotid endarterectomies. Detailed analyses of their carotid plaques included correlations between photographs of gross specimens, microscopic findings, angiograms, preoperative symptoms and lo...

High-resolution magnetic resonance imaging findings of basilar artery plaque in a patient with branch atheromatous disease: a case report

INTRODUCTION Intracranial branch atheromatous disease is a type of ischemic stroke that is caused by narrowing or occlusion of the orifice of the penetrating artery by atheromatous plaque. Pontine branch atheromatous disease is usually diagnosed using indirect findings such as the extension of a lesion to the basal surface of the pons because of the difficulty of demonstrating plaque in the bas...
