چکیده

While mortality from overt ischaemic heart disease is falling, that from heart failure is increasing and reaching epidemic proportions. Although an aging population is partly responsible for this trend, recent observations showing persistently high mortality and morbidity rates following myocardial infarction further compounds the issue. An understanding of the pathophysiological processes leading to heart failure, and in particular the mechanisms underlying postinfarction heart failure (postinfarction ventricular remodelling) is therefore fundamental and forms the basis of this review.

The role of autophagy emerging in postinfarction cardiac remodelling.

AIMS Autophagy is activated in cardiomyocytes in ischaemic heart disease, but its dynamics and functional roles remain unclear after myocardial infarction. We observed the dynamics of cardiomyocyte autophagy and examined its role during postinfarction cardiac remodelling. METHODS AND RESULTS Myocardial infarction was induced in mice by ligating the left coronary artery. During both the subacu...

Less heart is more.

The study by St John Sutton et al 1 in this issue of Circulation provides additional information on the importance of the myocardial substrate for the genesis of cardiac arrhythmias, in this instance, ventricular arrhythmias in the postinfarction remodeled human left ventricle (LV). The authors demonstrate significant relationships between ventricular arrhythmias and LV size, mass, and function...

Avascular Necrosis of the Femoral Head: Are Any Genes Involved?

Avascular necrosis of the femoral head (ANFH) is a pathologic process that results from interruption of blood supply to the femur bone resulting in the death of bone cells and collapse of the femoral head. Nontraumatic ANFH continuesto be a significant challenge to orthopedic surgeons. While the exact mechanisms remain elusive, many new insights have emerged from research in the last decade tha...

Hypereosinophilic Cardiac Involvement Presenting With Left Ventricular Massive Thrombus and Cardioembolic Stroke: A Case Report

Introduction: It is well known that the tendency toward thrombosis is increased in cancer patients. The increase in cancer procoagulant and tissue factor levels, endothelial damage, and stasis due to compression are among the most accused causes of thrombosis in cancer patients. Hypereosinophilia is a rare condition that causes endothelial damage leading to thrombosis. <s...

Raised diastolic pressure as an early predictor of left ventricular remodeling after infarction: should echocardiography or natriuretic peptides be used for assessment?

Full English text available from: www.revespcardiol.org “Risk” after myocardial infarction classically means risk of cardiac death or re-infarction; other adverse events can be included in the end-point such as hospitalization due to recurrent ischemia, new atrial fibrillation, embolic events, and others. However, the most important adverse “event” next to death and re-infarction is the develop...
