Prevalence and Mutational Patterns of Lamivudine-resistant Hbv Strains in Chronically Infected Serbian Patients

چکیده
This study aimed to determine the prevalence and mutational patterns of lamivudine-resistant hepatitis B virus (HBV) strains in chronically infected Serbian patients. The study included 154 patients on long-term lamivudine monotherapy. Resistance-associated mutations were identified by direct sequencing of the S/P gene. The genotypic resistance to lamivudine was confirmed in 54.5% of patients. Three primary resistance-associated mutations were found: rtM204V (55.9%), rtM204I (40.5%), rtA181T (3.6%) and two compensatory mutations rtV173L (17.8%) and rtL180M (67.8%). Seven mutational patterns were discovered with rtL180M+rtM204V being dominant (41.6%). The presence of resistanceassociated mutations was correlated to the older age of patients, the presence of clinically relevant HBsAg mutations and higher values of viral load. No correlation with HBV genotypes, subgenotypes or HBsAg subtypes was observed. High prevalence of resistance supports the use of genotypic testing in monitoring patients on lamivudine therapy and selecting those who would benefit from therapy with newly developed nucleos(t)ide analogs.

Evaluation of Lamivudine Resistance Mutations in HBV/HIV Co-infected Patients
Background and Objective: The drug resistance mutations are key elements in the failure of long-term treatment of Hepatitis B virus (HBV) and human immunodeficiency virus (HIV) infections. The mutation in the YMDD motif in the P gene of HBV is the most critical factor in antiviral drug (especially lamivudine) resistance. This study aimed to assess the YMDD motif and other polymerase gene mutati...
متن کاملHBV genotypes and antiviral-resistant variants in HBV infected subjects in Northern Italy.
HBV genotypes were investigated in sera/plasma from 97 HBV positive subjects. Genotype D was revealed in 80.4% followed by E in 6.2%. Genotypes A, B, and C were also found, as well as for the first time a new combination of HBV D and G genotypes. In a cohort of subjects of this population, the relationship with lamivudine and/or famciclovir-resistant HBV mutants was also investigated. Among 12 ...
متن کاملMutations in the Basal Core Promoter and Precore/Core Regions of Hepatitis B Virus in Patients Co-Infected With Human Immunodeficiency Virus
ABSTRACT Background and objectives: Globally, about one third of the population has been infected with Hepatitis B virus (HBV) and more than 400 million people have become chronically infected. Nearly, 20-25% of all carriers develop serious liver diseases such as cirrhosis, chronic hepatitis and hepatocellular carcinoma (HCC). According to t...
متن کاملHepatitis B virus variants in an HIV-HBV co-infected patient at different periods of antiretroviral treatment with and without lamivudine
BACKGROUND Lamivudine inhibits replication of both human immunodeficiency virus (HIV) and hepatitis B virus (HBV) and is commonly used as part of antiretroviral therapy. The main limitation in the use of lamivudine is resistant mutation selection. Most of these mutations affect the YMDD motif of the HBV DNA polymerase. The resistance occurs through M550V or M550I aminoacid replacements. The M55...
متن کاملIdentification of Drug Resistant Mutants of HBV (Hepatitis B Virus) by Direct Sequencing in Iranian Patients Treated with Lamivudine
Background and Aims: lamivudine is amongst the antiviral for drug chronic hepatitis B treatment. During therapy with lamivudine, variants may emerge with YMDD mutation in the reverse transcriptase (RT) region of polymerase gene. This mutation might have a role in drug resistant for HBV. Materials and Methods: HBV DNA extraction from serum sample of 88 patients, were subjected to nested PCR for ...
