Atherosclerotic renal artery stenosis, the oculostenotic reflex, and therapeutic nihilism.

چکیده
Fifteen years ago, Topol and Nissen1 described the oculostenotic reflex as an “irresistible temptation ... to perform angioplasty on any significant residual stenosis,” highlighting a widely held misperception that an angiographically severe stenosis must cause ischemia and that revascularization results in clinical benefit. Although quantitative angiography improves the accuracy of stenosis severity, it does not improve the accuracy of diagnosing ischemia. In fact, more than one third of angiographically severe coronary stenoses are hemodynamically insignificant by fractional flow reserve (FFR), and FFR results are highly correlated with findings of ischemia by myocardial perfusion imaging.2 Similarly, angiographic renal artery stenosis (RAS) severity correlates poorly with hemodynamic significance.3

Myocardial Bridges: Lessons in Clinical Coronary Pathophysiology.
M yocardial perfusion occurs primarily in diastole since systolic contraction transiently impedes coronary blood flow especially to the subendocardium. Thus, myocardial bridges replicate the normal microvascular physiology of high diastolic and low systolic flow, albeit at the level of the epicardial coronary artery. Because marked myocardial bridges are not common during invasive angiography, ...
متن کاملMITRAL STENOSIS COMPLICATED BY RENAL ARTERY EMBOLISM
Six patients with mitral stenosis complicated by renal artery embolism studied prospectively are presented. In five patients there was unilateral and in one patient bilateral renal artery involvement. The diagnosis was confirmed by intravenous pyelography, retrograde pyelography, renal angiography, renal scan or a combination of these. All had atrial fibrillation, proteinuria and elevated ...

Introduction: Renal artery stenosis is the most prevalent disease of renal artery and has an important role in making hypertension and renal atrophy. Since in previous researches, despite high frequency of risk of cardiovascular disease in Iran, there did not exist any general research investigating risk factors of coronary artery disease and incidence of contemporary renal artery stenisis, ...
متن کاملRenal artery stenosis: clinical and therapeutic implications.
Renal artery stenosis is a common cause of secondary hypertension with a rising incidence due to systemic atherosclerosis in an aging population. Patients with renovascular disease complicated by refractory hypertension, acute pulmonary edema, or progressive renal dysfunction benefit from aggressive medical therapy and may be considered for renal revascularization. Atherosclerotic renal artery ...
متن کاملPrevalence of renal artery stenosis in autopsy patients with stroke.
BACKGROUND AND PURPOSE Atherosclerotic renal artery stenosis commonly exists as one manifestation of more generalized atherosclerosis. It is a progressive but potentially curable disorder. Thus, information on renal artery involvement in atherosclerotic diseases could be important. We investigated the prevalence of renal artery stenosis in autopsied patients with stroke over 40 years of age. ...
