Acute aortic occlusion in a patient with chronic paralysis due to spinal cord injury: a case report

چکیده
Patients with spinal cord injury experience changes in the cardiovascular system and a high morbidity associated with peripheral artery disease. We report a case of acute aortic occlusion in a patient with chronic paralysis due to spinal cord injury. A 65-year-old man with chronic paralysis due to spinal cord injury developed mottling of the right extremity. Because of the complete tetraplegia, the patient had no subjective symptoms. Computed tomography revealed occlusion of the infrarenal abdominal aorta. An emergency thromboembolectomy established adequate blood flow, and the postoperative course was uneventful. The loss of muscle mass might be an advantage in avoiding ischemia reperfusion syndrome. Early detection of acute aortic occlusion and immediate reperfusion are primarily important, but patients with chronic paralysis present a risk of delay in detection, diagnosis, and treatment of acute aortic occlusion because of motor or sensory deficits. Although rare, it is necessary to consider acute aortic occlusion in the case of acute limb ischemia in patients with chronic paralysis due to spinal cord injury.

Preoperative stress conditioning prevents paralysis after experimental aortic surgery: increased heat shock protein content is associated with ischemic tolerance of the spinal cord.
BACKGROUND All forms of surgical therapy are stressful and injurious. The problems of paralysis, renal dysfunction, and colonic ischemia associated with aortic occlusion are due to acute ischemia-reperfusion injury at the cellular level. Acute-anterior spinal cord ischemia is the most devastating outcome of these iatrogenic-ischemic events. The majority of surgical procedures are performed elec...

Acute Flaccid Myelitis (AFM), a polio-like paralysis in children no older than 21, has recently reemerged and increasing numbers of such cases have been reported worldwide since 2012. Accurate and early diagnosis of this condition could help with better management of the disease. A 9- year- old girl with chief complaint of headache, fever and vomiting was subsequently affected by an acute...

Introduction: Postoperative neurological deficit is the most devastating complication after thoracoabdominal aortic aneurysm repair. Despite demonstrated neuroprotective effects of estradiol, its protective efficacy against spinal cord ischemia-reperfusion and underlying mechanisms are not yet elucidated. Methods: Two groups, each of 10 New Zealand white male rabbits, were studied. Control g...

Introduction: Various neuroregenerative procedures have been recently employed along with neurorehabilitation programs to promote neurological function after Spinal Cord Injury (SCI), and recently most of them have focused on the acute stage of spinal cord injury. In this report, we present a case of acute SCI treated with neuroprotective treatments in conjunction with conventional reha...
