Cost-benefit analysis of Helicobacter pylori screening

ثبت نشده
چکیده

Health technology The study compared two intervention strategies for the management of Helicobacter pylori (Hp). Strategy 1 was population-based Hp screening with subsequent treatment for all positive screened individuals. Strategy 2 was no screening, with the testing and treatment of Hp only if associated clinical symptoms were documented. Screening was performed using an enzyme immunoassay to investigate serum immunoglobulin (Ig)G and IgA antibodies to Hp. Lower limits of raised titres were 700 for IgG and 70 for IgA antibodies. Treatment, which was administered for 1 week, consisted of amoxycillin (1,000 mg x 2), clarithromycin (500 mg x 2) and omeprazole (20 mg x 2).

Cost-benefit analysis of Helicobacter pylori screening.

BACKGROUND Helicobacter pylori screening may markedly reduce mortality and morbidity in the decades ahead. AIMS This study explores the costs and benefits of population-based H. pylori screening in terms of health care cost taking into account all relevant H. pylori-related diseases. MATERIAL AND METHODS The computer-based decision analysis compared two strategies: (1) screen for H. pylori ...

Helicobacter pylori and gastric cancer: what are the benefits of screening only for the CagA phenotype of H. pylori?

BACKGROUND Strains of Helicobacter pylori that express the CagA protein are associated with a threefold increased gastric cancer risk as compared to H. pylori strains that do not express CagA. Screening and treatment only for CagA antibodies should target those individuals at highest gastric cancer risk while reducing the number of patients requiring antibiotics. We compared the costs and benef...

Cost-benefit analysis of testing for Helicobacter pylori in dyspeptic subjects.

OBJECTIVES It has remained controversial how to manage patients who present with symptoms of dyspepsia. Should all dyspeptic patients undergo a radiological or endoscopic procedure to diagnose peptic ulcer, or would a serological test for Helicobacter pylori provide sufficient evidence to start all H. pylori-positive patients on empirical antibiotic therapy? METHODS The outcome of serological...

Clinical and economic effects of population-based Helicobacter pylori screening to prevent gastric cancer.

BACKGROUND Helicobacter pylori infection has been identified as a risk factor for certain types of gastric cancer. However, the extent to which H. pylori eradication decreases the risk of gastric cancer is unknown, raising the question of whether population-based H. pylori screening should be undertaken. OBJECTIVE To compare clinical and economic effects of H. pylori screening, with and witho...

A screening program to test and treat for Helicobacter pylori infection: Cost-utility analysis by age, sex and ethnicity

BACKGROUND The World Health Organization recommends all countries consider screening for H. pylori to prevent gastric cancer. We therefore aimed to estimate the cost-effectiveness of a H. pylori serology-based screening program in New Zealand, a country that includes population groups with relatively high gastric cancer rates. METHODS A Markov model was developed using life-tables and morbidi...
