The Effect of Cortical Strut Grafting on Femoral Strain in Cemented and Noncemented Hip Arthroplasty

چکیده
Introduction: One possible cause of thigh pain following noncemented total hip arthroplasty is increased stress in the mid-portion of the femur. Anterolateral placement of a cortical strut allograft has successfully alleviated thigh pain in symptomatic patients. The purpose of this study was to show experimentally whether the surface strains on the femur are altered by the presence of cemented and noncemented implants and by the addition of the strut graft. Cemented implants, which typically are not associated with thigh pain, were included as a control group.

Reconstruction of femoral defects in revision hip surgery: risk of fracture and stem migration after impaction bone grafting.
The use of impaction bone grafting during revision arthroplasty of the hip in the presence of cortical defects has a high risk of post-operative fracture. Our laboratory study addressed the effect of extramedullary augmentation and length of femoral stem on the initial stability of the prosthesis and the risk of fracture. Cortical defects in plastic femora were repaired using either surgical me...

Finite-element analysis was used to study the general differences in load-transfer mechanisms and stress patterns of cemented, fully ingrown, proximally ingrown, and smooth press-fitted femoral stems in total hip arthroplasty (THA). Identical stems were used for the noncemented configurations and a similar stem shape for the cemented configurations. In each model, bone properties and loading ch...
متن کاملImpaction Grafting for Revision Total Hip Arthroplasty
Impaction grafting for revision total hip arthroplasty is increasingly being used in cases complicated by substantial bone loss. Many techniques using cemented and noncemented designs with or without bone graft have had unpredictable outcomes and are the source of continuing debate. This new technique uses morselized allograft, impacted into a constrained femoral envelope, creating a new medull...

Total hip replacement has evolved into one of the most frequently performed reconstructive procedures in orthopaedic surgery today. A modified anterior approach developed by the senior surgeon has been utilized in performing over 3,000 operations in the past two decades. It is a unique and facile exposure. It can be easily applied to primary and revision surgery. Noncemented prostheses have bee...

OBJECTIVE To determine the prevalence of heterotopic bone formation in cemented versus noncemented total hip joint replacement. DESIGN A prospective randomized controlled trial. Follow-up ranged from 2 to 6 years (mean 4 years). SETTING A university hospital. PATIENTS Two hundred and twenty-six patients who had primary or secondary osteoarthrosis of the hip were stratified according to ty...
