Bedside-measurements of electrical impedance tomography and functional residual capacity during positioning therapy in a case of acute respiratory failure

چکیده
The lungs of patients with Acute Respiratory Failure (ARF) are characterized by reduced lung volume and regional heterogeneity of ventilation distribution. To improve lung volume and ventilation distribution, protective mechanical ventilation with positive end-expiratory pressure (PEEP), recruitment maneuvers, and some adjunct measures like intermittent proning have been recommended. But the continuous monitoring and management of these measures is not possible at bedside. New non-invasive monitoring tools like Electrical Impedance Tomography (EIT) and measurements of Functional Residual Capacity (FRC) by oxygen wash-in/washout have been advocated, but are not established in clinical routine. Thus, evident therapy algorithms are not yet available. The aim of this case study was to describe the ability of EIT and FRC measurement to detect changes in regional ventilation and lung volume in an ARF patient, who was treated by alveolar recruitment maneuver and intermittent prone position. An increase in FRC was observed in parallel of gas exchange improvement immediately after the recruitment. During prone position, FRC decreased constantly while a redistribution of ventilation among dorsal lung regions was detected by EIT. These two tools were compared but without a gold standard. The reported case shows that EIT and FRC measurements are two different methods, which are difficult to compare. As they do not present a good correspondence, the respective results may be under or over estimated but the measures indicated some tendencies. Nevertheless the combination of both could be a meaningful monitoring supplementation in the respiratory therapy of ARF patients.

Regional overdistension during prone positioning in a patient with acute respiratory failure who was ventilated with a low tidal volume: a case report
Background Prone positioning may provide a uniform distribution of transpulmonary pressure and contribute to prevent ventilator-induced lung injury. However, despite moderate positive end-expiratory pressure and low tidal volumes, there is still a risk of regional overdistension. Case presentation A man with refractory hypoxemia was mechanically ventilated with prone positioning. Although pro...
متن کاملUse of thoracic electrical impedance tomography as an auxiliary tool for alveolar recruitment maneuvers in acute respiratory distress syndrome: case report and brief literature review
Thoracic electrical impedance tomography is a real-time, noninvasive monitoring tool of the regional pulmonary ventilation distribution. Its bedside use in patients with acute respiratory distress syndrome has the potential to aid in alveolar recruitment maneuvers, which are often necessary in cases of refractory hypoxemia. In this case report, we describe the monitoring results and interpretat...
متن کاملAutomatic protective ventilation using the ARDSNet protocol with the additional monitoring of electrical impedance tomography
INTRODUCTION Automatic ventilation for patients with respiratory failure aims at reducing mortality and can minimize the workload of clinical staff, offer standardized continuous care, and ultimately save the overall cost of therapy. We therefore developed a prototype for closed-loop ventilation using acute respiratory distress syndrome network (ARDSNet) protocol, called autoARDSNet. METHODS ...

Background: We measured Functional Residual Capacity (FRC) of the lungs with three methods in healthy volunteers. The three techniques included a CO2 partial rebreathing technique, nitrogen washout technique, and the reference technique for ambulatory patients, body plethysmography. Materials and Methods: After granting consent to an IRB-approved protocol, each of the 20 healthy volunteers part...
متن کاملElectrical impedance tomography (EIT) for quantification of pulmonary edema in acute lung injury
BACKGROUND Assessment of pulmonary edema is a key factor in monitoring and guidance of therapy in critically ill patients. To date, methods available at the bedside for estimating the physiologic correlate of pulmonary edema, extravascular lung water, often are unreliable or require invasive measurements. The aim of the present study was to develop a novel approach to reliably assess extravascu...
