Challenge of conducting a placebo-controlled randomized efficacy study for influenza vaccine in a season with low attack rate and a mismatched vaccine B strain: a concrete example
نویسندگان
چکیده
BACKGROUND Our aim was to determine the efficacy of a trivalent inactivated split virus influenza vaccine (TIV) against culture-confirmed influenza A and/or B in adults 18 to 64 years of age during the 2005/2006 season in the Czech Republic. METHODS 6203 subjects were randomized to receive TIV (N = 4137) or placebo (N = 2066). The sample size was based on an assumed attack rate of 4% which provided 90% power to reject the hypothesis that vaccine efficacy (VE) was > or = 45%. Cases of influenza like illness (defined as fever (oral temperature > or =37.8 degrees C) plus cough and/or sore throat) were identified both by active (biweekly phone contact) and passive (self reporting) surveillance and nasal and throat swabs were collected from subjects for viral culture. RESULTS TIV was well tolerated and induced a good immune response. The 2005/2006 influenza season was exceptionally mild in the study area, as it was throughout Europe, and only 46 culture-confirmed cases were found in the study cohort (10 influenza A and 36 influenza B). Furthermore among the B isolates, 35 were identified as B/Hong Kong 330/2001-like (B/Victoria/2/87 lineage) which is antigenically unrelated to the vaccine B strain (B/Yamagata/16/88 lineage). The attack rate in the vaccine group (0.7%) was not statistically significantly different from the attack rate in the placebo group (0.9%). CONCLUSION Due to the atypical nature of the influenza season during this study we were unable to assess TIV efficacy. This experience illustrates the challenge of conducting a prospective influenza vaccine efficacy trial during a single season when influenza attack rates and drift in circulating strains or B virus lineage match can be difficult to estimate in advance. TRIAL REGISTRATION Clinical trial registery: NCT00197223.

Virus shedding after H9N2 strain challenge of SPF Chickens immunized by an experimental Nano-Adjuvant and commercial oil emulsion avian influenza (H9N2) vaccines
Abstract Background and Aims: Vaccination of poultry has a major impact on the prevention and control of avian influenza viruses. Nanobiotechnology techniques provide a new approach for improvement of influenza vaccine efficacy. In this study, efficacy of an inactivated nano-adjuvant vaccine developed based on an endemic H9N2 virus was evaluated in SPF chickens. Materials and Methods: I...
متن کاملEvaluation the Efficacy of Anthrax Vaccine against Challenge with a Highly Virulent Strain of Bacillus anthracis Isolated from Soil in Sheep, Goats and Guinea Pigs in Iran
Protection of animals immunized against Bacillus anthracis is usually demonstrated by challenging with an appropriate dose of a strain of Bacillus anthracis that is lethal to unvaccinated animals inoculated at the same time. In this study the protective efficacy in anthrax vaccine (34F2 sterne strain spore) was evaluated in sheep, goats and guinea pigs challenged with subcutaneous inoculation...

Background and Aims: Influenza vaccination is one of the best way to prevent and control influenza worldwide. It is manufactured by WHO-licensed companies based on the WHO expertise committee annually. The aim of this study was partial quality control of the commercial human influenza vaccine 2008-9 and its matching with the circulating strains. Materials and Methods: The trivalent imported vac...
متن کاملEffect of influenza vaccines against mismatched strains: a systematic review protocol
BACKGROUND Influenza vaccines are most effective when the antigens in the vaccine match those of circulating influenza strains. The extent to which the vaccine is protective when circulating strains differ from vaccine antigens, or are mismatched, is uncertain. We propose to systematically review the cross-protection offered by influenza vaccines against circulating influenza A or B viruses tha...
متن کاملMonitoring virus harvesting time in embryonated chicken eggs inoculated with avian influenza H9N2 vaccine strain
Knowledge of virus and replication kinetic is one of the most important issues in the vaccine production. The present study aimed to evaluate the best harvesting time of H9N2 avian influenza virus (AIV) vaccine strain inoculated in specific pathogen free (SPF) embryonated chicken eggs (ECE)s. For this purpose, 10-5 dilution of AIV (A/Chicken/Iran/99/H9N2) was inoculated into 336, 11-day old ECE...
