چکیده

2015 The complexity of ischemic heart disease (IHD) comprehends disease in the succeeding perfusion domains of the vascular tree. Fractional flow reserve and coronary flow reserve are validated diagnostic modalities to identify myocardial ischemia, but solitarily do not suffice to objectify the respective contribution of obstructive and nonobstructive disease to IHD. Combined pressure and flow measurements deliver a comprehensive intracoronary assessment of IHD, although fractional flow reserve and coronary flow reserve disagree in 40% of the cases. Discrepancy between the indices does not reflect methodological failure, but explores divergent extremes of epicardial and microvascular disease, which holds vital prognostic value. We advocate critical revision of the current diagnostic and therapeutic approaches toward IHD. In this review, we deliver a perspective on the future developments in the diagnosis and treatment of IHD.

Assessment of left ventricular torsion in long axis view in patients with chronic stenosis of coronary arteries

Background: Left ventricular torsion is one of the most important biomechanical parameters of heart that routinely is measured in short axis view.  A review of the literature has indicated that assessment of left ventricular torsion in short axis view has some limitations. In the present study, we evaluated whether torsion angle assessment in long axis view can be used as a diagnostic biomechan...

Detection of Autoregulatory Changes in Nonstenotic Coronary Arteries Induced by Collateral Flow to Adjacent Stenotic Arteries

Background Our study compares the effect of acute proximal stenosis of a coronary artery supplying a myocardial perfusion bed with that of stenosis of an adjacent artery resulting in collateral flow diversion supplied by the same perfusion bed. These alterations in coronary physiology were quantified by digital angiographic impulse response analysis of contrast material mean transit time for th...

Magnetic resonance imaging of the microcirculation in acute coronary syndromes

Microvascular damage is frequently observed in the setting of acute coronary syndromes (ACS). Imaging of the microcirculation is playing an increasingly important role in the risk stratification of patients presenting with acute coronary syndromes, furthering our understanding about coronary microcirculatory patho-physiology in this setting. Of the available imaging methods, cardiac magnetic re...

بررسی ذخیره آهن در بیماران تحت آنژیوگرافی عروق کرونر

Background: Epidemiological studies have suggested an association between higher body iron stores and coronary artery disease. It is believed that inflammation and oxidation are important mechanisms involved in the complex pathological process of atherogenesis. Free radical production is catalyzed and accelerated in the presence of iron. The determination of plasma ferritin levels is a convenie...

Digital angiographic assessment of the physiological changes to the regional microcirculation induced by successful coronary angioplasty.

BACKGROUND Impulse response analysis of digital coronary angiographic images calculates a parameter known as the mean transit time of the microcirculation (Tmicro). This has been shown to accurately assess the regional microcirculatory response to proximal stenosis in relation to flow. Our goal was to apply impulse response analysis to patients undergoing successful angioplasty and to quantify ...
