Ethacrynic Acid Effectiveness and Mode of Diuretic Action
نویسنده
چکیده
ETHACRYNIC ACID is an unsaturated ketone derivative of phenoxyacetic acid* chemically different from other known diuretic agents.' Compounds of this general class have a significant ability to bind sulfhydryl groups in vitro. Ethacrynic acid was shown by Baer and associates2 to be highly effective as a diuretic in the dog, but not in the rat. A preliminary report from this laboratory indicated that the compound was a highly potent natriuretic and diuretic agent in man.3 This study also suggested that ethacrynic acid had a unique mode of action in the nephron. In the present communication results of a 2-year experience with ethacrynic acid are reported. During this period ethacrynic acid was found to produce effective natriuresis and diuresis in a study of 82 patients, many of whom had edema which was refractory to treatment with other diuretic agents. The results of a series of investigations designed to explore some of the mechanisms by which the compound alters electrolyte excretion and acid-base balance are also reported.

متن کاملHypotensive Effect of Ethacrynic Acid.
THE benzothiadiazine derivatives were the first diuretic compounds that could safely be administered over considerable periods of time and yet retain their effectiveness. In the course of the investigation of their diuretic properties in man it became apparent that they also possessed weak antihypertensive properties.' The mechanism of this action remains uncertain, and there is doubt as to whe...
متن کاملDiuretic Renal Scintigraphy in Patients with Sulfonamide Allergies: Possible Alternative Use of Ethacrynic Acid.
UNLABELLED Furosemide may trigger life-threatening sulfonamide cross-hypersensitivity reactions, posing a dilemma in patients who need diuretic renal scintigraphy. METHODS We present our experience using ethacrynic acid as a nonsulfonamide alternative diuretic in 5 patients, with a discussion of the diuretic molecular structure, potential side effects, protocol development, and imaging result...
متن کاملRole of macula densa in diuretics-induced renin release.
Diuretic therapy may enhance renin release by various mechanisms, principally contraction of extracellular fluid volume and its effects, including a fall in arterial pressure. Awake hydropenic or volume-expanded rats received diuretics (amiloride and hydrochlorothiazide) that are known inhibitors of NaCl transport beyond the macula densa; also the well-known Na(+)-K(+)-2 Cl- transport system in...

1. The renal excretion of phosphate was studied in dogs under control conditions and during infusion of ethacrynic acid or frusemide at different levels of filtered load. 2. Both of these diuretic compounds significantly enhanced the fractional excretion of phosphate at normal and moderately decreased filtered loads, and a good correlation between fractional excretions of sodium and phosphate c...
