Rising caesarean section rates: a cause for concern?
نویسندگان
چکیده
Over the past 25 years, there has been a sustained increase in caesarean section rates in the developed world, with massive public interest, and debate, on both the causes and appropriateness of increasingly employing a surgical procedure to short circuit, or entirely bypass, labour and delivery. This debate has largely centred on the operative risks to the mother, the increased respiratory morbidity in the infant, the increased use of expensive hospital resources balanced against the reality of defensive obstetrics, maternal request, operator convenience and remuneration. A frequently quoted World Health Organisation consensus statement examined caesarean section rates and maternal and perinatal mortality rates in a number of countries and concluded that there were no additional health benefits associated with a caesarean section rate above 10% to 15%. This concept has formed the impetus for a recent national audit of caesarean section rates in the UK. However, the use of crude perinatal mortality rates may mask any benefit from caesarean section, as the main contributors to crude perinatal mortality rates, lethal anomalies and low birthweight, are not readily amenable to obstetric intervention and unlikely to be affected by caesarean section. This study re-examines the relationship between caesarean section rates and birthweight-specific perinatal mortality rates in three large maternity hospitals in Dublin over a 22 year period.

Caesarean section rates in government hospitals in Malaysia, 2000-2001.
There has been widespread concern about rising caesarean section rates in the world. Various reviews have documented the wide variation worldwide in caesarean rates. For instance, in the mid-to-late 1980s the caesarean rate was 32% in Brazilian states, 25% in the United States, 5% in Jamaica and 7% in Czechoslovakia!. It is believed that the rising trend is due to the general trend towards lowe...

Despite the World Health Organization’s (WHO) recommendation that caesarean section (c-section) rates should not exceed 15% [1], the high rates in some countries are cause for concern. For example, Italy, China, Mexico and Brazil all have rates higher than 36% [2] with great variation within each nation. The need for c-section has probably increased for many reasons, including rising rates of o...
متن کاملRising caesarean section rates in public hospitals in Malaysia 2006.
The caesarean section rate in Malaysian public hospitals has increased to 15.7% from 10.5% in the year 2000. There are inter-state variations in the rate ranging from a high of 25.4% in Melaka to 10.9% in Sabah. The West Coast states generally had a higher caesarean section rate than the East Coast states as well as East Malaysia. It would be prudent for Malaysia to implement stringent caesarea...
