چکیده

Over the past 12 years, catheter ablation of atrial fibrillation (AF) has emerged from being a novel, largely unproven procedure to its role today as a very commonly performed procedure for treatment of patients with symptomatic AF.1–4 In fact, catheter ablation of AF has become one of the most commonly performed catheter ablation procedures at most large medical centers throughout the world. The objective of the present report is to provide a state-of-the-art review of the techniques, outcomes, and complications of catheter ablation of AF. Three main topic areas will be discussed. First, I will review the current approaches for and outcomes of AF ablation. Second, I will identify and briefly review areas in the field of AF ablation about which less is known and that remain active areas for research. Third, I will briefly review the current status of a number of emerging technologies, approaches, and tools that are being developed with the aim of improving the outcomes of AF ablation. Finally, I will conclude by sharing with you my perspective on the indications for AF ablation.

Cardiac Remodeling After Atrial Fibrillation Ablation.

Radiofrequency catheter ablation procedures are considered a reasonable option for patients with symptomatic, drug refractory atrial fibrillation (AF). Ablation procedures have been reported to effectively restore sinus rhythm and provide long-term relief of symptoms. Both electrical and structural remodeling occurs with AF. A reversal of the electrical remodeling develops within 1 week after r...

Cardiac Resynchronization Therapy and Atrial Fibrillation.

Cardiac resynchronization therapy (CRT) is an important advance for the treatment of end--stage heart failure (HF). About 15-50% of HF is complicated by atrial fibrillation (AF) and associated with worsened outcomes. Meta-analyses from observational studies suggest that patients with AF derive similar benefits to CRT as patients in sinus rhythm (SR). The presence of AF, however, may interfere w...

Recent advances in rhythm control for atrial fibrillation

Atrial fibrillation (AF) remains a difficult management problem. The restoration and maintenance of sinus rhythm-rhythm control therapy-can markedly improve symptoms and haemodynamics for patients who have paroxysmal or persistent AF, but some patients fare well with rate control alone. Sinus rhythm can be achieved with anti-arrhythmic drugs or electrical cardioversion, but the maintenance of s...

Time to cardioversion of recurrent atrial arrhythmias after catheter ablation of atrial fibrillation and long-term clinical outcome.

INTRODUCTION It is unclear whether early restoration of sinus rhythm in patients with persistent atrial arrhythmias after catheter ablation of atrial fibrillation (AF) facilitates reverse atrial remodeling and promotes long-term maintenance of sinus rhythm. The purpose of this study was to determine the relationship between the time to restoration of sinus rhythm after a recurrence of an atrial...

A new approach for catheter ablation of atrial fibrillation: mapping of the electrophysiologic substrate.

OBJECTIVES We sought to test the hypothesis that complex fractionated electrograms (CFAEs) recorded during atrial fibrillation (AF) could be used as target sites for catheter ablation of AF. BACKGROUND Mapping of AF in humans has shown that areas of CFAEs correlate with areas of slowed conduction and pivot points of reentrant wavelets. We hypothesized that such areas of CFAEs could be identif...

Strain rate imaging for functional quantification of the left atrium: atrial deformation predicts the maintenance of sinus rhythm after catheter ablation of atrial fibrillation.

AIMS The aim of the study was to investigate the atrial myocardial deformation properties using ultrasound strain rate (SR) imaging in patients after catheter ablation of atrial fibrillation (AF) and to compare its prognostic value in maintaining sinus rhythm. METHODS AND RESULTS A total of 118 patients with AF (74 paroxysmal AF, 44 persistent AF) underwent transthoracic echocardiography with...
