J Bone Joint Surg [Br] 2008;90-B:1473-6. Received 3 January 2008; Accepted after revision 27 June 2008 Our aim in this prospective radiological study was to determine whether the flexibility rate calculated from radiographs obtained during forced traction under general anaesthesia, was better than that of fulcrum-bending radiographs before corrective surgery in predicting the extent of the available correction in patients with idiopathic scoliosis. We evaluated 33 patients with a Cobb angle > 60° on a standing posteroanterior radiograph, who had been treated by posterior correction. Pre-operative standing fulcrum-bending radiographs and those with forced-traction under general anaesthesia were obtained. Post-operative standing radiographs were taken after surgical correction. The mean forced-traction flexibility rate was 55% (SD 11.3) which was significantly higher than the mean fulcrum-bending flexibility rate of 32% (SD 16.1) (p < 0.001). We found no correlation between either the forced-traction or fulcrum-bending flexibility rates and the correction rate post-operatively (p = 0.24 and p = 0.44, respectively). Radiographs obtained during forced traction under general anaesthesia were better at predicting the flexibility of the curve than fulcrum-bending radiographs in curves with a Cobb angle > 60° in the standing position and may identify those patients for whom supplementary anterior surgery can be avoided.

The Usefulness of Employing an Electronic Traction Table to Determine Flexibility in Adolescent Idiopathic Scoliosis.
AIM The aim of the study was to develop new equipment for the assessment of the flexibility of the spine with different forces. This new system should provide a different perspective to adolescent idiopathic scoliosis (AIS) for the selection of fusion levels and surgical success. MATERIAL AND METHODS Eighteen patients suffering from AIS who were scheduled to undergo posterior instrumented spi...
متن کاملCurve flexibility in cerebral palsy-related neuromuscular scoliosis: does the intraoperative prone radiograph reveal more flexibility than preoperative radiographs?
BACKGROUND Spinal flexibility is determined preoperatively by manipulating the spine and assessing, radiographically, to what extent the amount of deformity reduces. Quantifying spinal flexibility is important when determining the approach to the planned operation in order to achieve the most optimal spinal correction and balance. Currently, supine traction radiography is a popular method used ...

Aim: To determine how accurately pre-operative stress radiographs predict the final outcome in adolescent idiopathic scoliosis surgery. Methods: Records of 20 patients were reviewed retrospectively. Pre-operative correction was measured by comparing the initial Cobb angle of the main structural curves on plain standing radiographs to values measured at corresponding levels after correction on t...
متن کاملEffects of Specific Schroth Exercises on Cobb Angle, Flexibility, and Functional Capacity of Adolescents with Idiopathic Scoliosis
Background and Objective:Adolescent idiopathic scoliosis is one of the complex and common deformities of the spine. It is a condition that can cause a lot of pain and discomfort for individuals and has consequences and complications. There are different exercises and approaches for correction of this deformity, such as Schroth exercise. In this regard, the purpose of this study was to investiga...
متن کاملEvaluating the Rotation Correction of the Main Thoracic Curve in Severe Adolescent Idiopathic Scoliosis: Bending and Traction vs. Fulcrum – A Preliminary Report
BACKGROUND Flexibility evaluation methods were only used to assess the changes of coronal Cobb angle in patients with adolescent idiopathic scoliosis (AIS). Little attention was attached to the vertebral rotation in these methods. MATERIAL AND METHODS 21 patients with severe adolescent idiopathic scoliosis were enrolled in this study. Coronal flexibility and rotation correction were compared on...
