Hearing and seeing: Unusual early signs of Wernicke encephalopathy.

چکیده
A 31-year-old woman with Crohn disease presented with tinnitus, headache, apathy, blurred vision, and nystagmus after 3 weeks of IV nutrition and partial colectomy. MRI showed abnormalities of the inferior colliculi (figure). Serum thiamine was low (56 nM/L, normal 66) and symptoms resolved within 48 hours of thiamine IV supplementation, suggestive of Wernicke encephalopathy (WE). MRI usually shows damage of the mammillary bodies and paraventricular region. Hearing loss has been reported in WE,1 but not in relation with a lesion of the inferior colliculi, which is the first to occur in a nonhuman primate model of thiamine deficiency.2

Wernicke's encephalopathy presenting with severe dysphagia: a case report.
A 62-year-old man developed dysphagia 4 weeks before the classic symptoms and signs of Wernicke's encephalopathy appeared. Adequate treatment with parenteral thiamine resulted in complete recovery of all symptoms, including his dysphagia. This extraordinary presentation with dysphagia, and the unusual course of the disease, encouraged us to present this case history.
متن کاملUnusual Presentation of Uncommon Disease: Anorexia Nervosa Presenting as Wernicke-Korsakoff Syndrome—A Case Report from Southeast Asia
Anorexia nervosa presenting as Wernicke-Korsakoff syndrome is rare. We report a case of anorexia nervosa, who presented with Wernicke's encephalopathy and prog...
متن کاملProton MR spectroscopy in Wernicke encephalopathy.
Wernicke encephalopathy is caused by thiamine deficiency. Although the clinical picture has been well established for some time, clinical diagnosis is attained in only 20% of the cases. MR imaging techniques contribute to early diagnosis of Wernicke encephalopathy. We herein report MR imaging and proton MR spectroscopic findings for a patient with clinical and biochemical features consistent wi...
