Polycythemia vera (PV), essential thrombocythemia (ET), and primary myelofibrosis (PMF) are chronic myeloproliferative neoplasms (MPN) characterized by clonal expansion of an abnormal hematopoietic stem/progenitor cell. Their natural history is marked by thrombohemorrhagic complications and a propensity to transform into myelofibrosis and acute leukemia. The understanding of the MPN pathophysiology dramatically improved following the description of recurrent molecular abnormalities, mainly represented by the V617F mutation in JAK2 exon 14, which involves.95% of PV and ;60% to 70% of ET and PMF patients. The aim of this paper is to update the incidence and risk factors, the mechanisms of thrombogenesis, and the management recommendations of vascular complications in MPN.
